Overview

When someone goes into cardiogenic shock, their heart stops pumping blood like it’s supposed to. This typically happens during a heart attack, but other problems can also cause this life-threatening condition.

When blood doesn’t reach the organs in a normal way, they can shut down. People who are in cardiogenic shock might even temporarily need special devices to keep them alive. These devices are known as temporary mechanical circulatory support.

The Center for Shock and Circulatory Support (CSCS) at Cleveland Clinic brings together several teams to care for patients with cardiogenic shock. The goal is to standardize care and help coordinate complex care that stretches across teams. It’s comprised of the CCF Main Campus Cardiogenic Shock Team, Circulatory Support Technical Services and Shock Research/Innovations.

What We Treat

Our CSCS teams focus exclusively on helping healthcare providers optimize outcomes for patients in cardiogenic shock.

Healthcare providers should feel comfortable reaching out to the CSCS team if someone they’re treating needs temporary mechanical circulatory support, or has major heart issues that haven’t responded to other types of treatment.

Using mechanical circulatory support devices can benefit someone if they:

• Have any kind of cardiogenic shock.
• Have advanced heart failure.
• Need extracorporeal life support-cardiopulmonary resuscitation (ELCS-CPR).

The mechanical circulatory support platforms supported at Cleveland Clinic’s main campus include:

• Durable ventricular assist devices, including but not limited to:
  • HeartMate 3^®.
  • HeartMate II^® (Maintenance support only).
  • HeartWare^® (Maintenance support only).

• Temporary mechanical circulatory support devices, including but not limited to:
  • Abiomed Impella^® suite of devices.
  • TandemHeart^®
  • Intra-aortic balloon pump.
  • Veno- arterial extracorporeal membrane oxygenation (ECMO).

• Other Devices, including but not limited to:
  • Evolving Investigational Device Exemption (IDE) devices.

The center’s mission focuses on myocardial recovery. It leverages the use of temporary circulatory support devices to bridge patients to more definitive state-of-the-art therapies. These therapies can include high risk coronary interventions or coronary artery bypass grafting, valvular interventions, or high risk ablation. Timely bridging to more durable left ventricular assist devices (LVAD) and heart transplantation can be considered in cases that do not recover. As the field continues to evolve, new investigational therapies will become available.
