The Diabetes Center provides patients access to a multidisciplinary team, including endocrinologists, diabetes educators, a pharmacist and a nurse practitioner.
The goal of the Diabetes Center is to encourage patients to receive early specialty care and education — getting them on the right track with their diabetes management before returning them to their primary care physicians for ongoing management. The Diabetes Center offers both individual and group diabetes education classes year round for patients and their families.

Help for High-risk Patients with Diabetes
Patients with difficult-to-control diabetes or high-risk patients with diabetes, such as those who have recently experienced a heart attack or underwent bypass surgery, can receive intensive educational courses to help them better manage their disease. The goal of this training is to help patients stabilize their blood sugar levels within six months. Most patients are returned back to their primary care providers within 12 months.

Virtual visits
You can now stay connected to your healthcare team through virtual visits, using your smartphone, tablet or computer.
Why go virtual? It's an easy, convenient and secure way to see your provider face-to-face without having to leave home. This saves you travel time, parking fees and time spent in the waiting room — and you can also have a loved one or caregiver join you. If appropriate, you can also get a prescription sent to the pharmacy of your choice.

Comprehensive diabetes self-management education
Diabetes Self-Management Education and Support programs offer comprehensive diabetes self-management education to help you or your loved one succeed in managing all aspects of diabetes care.
Group classes and individual visits are available, and both are taught by diabetes care and education specialists. Register now to take one of the following courses close to home or work. A physician referral is required.
- Comprehensive Diabetes Education: Class topics include eating healthfully, being physically active, monitoring blood sugars and interpreting results, taking medications safely, reducing the risk of complications, and setting personal goals for managing diabetes.
- Introduction to Insulin Pumps: This course provides education about insulin pumps, and will cover topics such as the benefits of pumping, insulin pump limitations, and types of pumps and infusion sets. Attendees will get hands-on time learning about the different pumps. This class is also for current pump users who are considering other pump options or who need to refresh their basic knowledge of pump management.
- Weight Management Program: Classes in this program teach healthy eating in a group atmosphere and cover topics including balancing diets, dining out, shopping tips, recipes and menus, weight plateaus, emotional and social eating, fiber and fit, time savers, and exercise.

Diabetes Technology
Diabetes technology has advanced remarkably. Your diabetes healthcare should be the focus with the advent of each new technology. At Cleveland Clinic Endocrinology & Metabolism Institute, we offer several clinics and classes to match your unique healthcare needs and available technology such as continuous glucose monitors (CGM), insulin pumps, and smart pens.
What is a continuous glucose monitor (CGM)?
A CGM is a device that helps you to monitor your glucose level continuously. In most cases, a CGM sensor is inserted under the skin using a needle. The needle is removed leaving only the sensor under the skin. The sensor is usually replaced every 10-14 days by the user.
While wearing a CGM, sensor glucose readings are sent wirelessly to a device that will show you the readings (either a smartphone, an insulin pump, or a receiver). The glucose information includes the current level and whether the level is expected to go up, down, or hold steady. High and low limits can be set to alert the user to glucose levels outside their target range.
What is an insulin pump?
An insulin pump is a small, computerized device, about the size of a small cell phone. Insulin pumps can be worn on your belt, in your pocket, or under your clothes. Insulin pumps deliver rapid-acting insulin 24 hours a day through a small flexible tube called a cannula. The cannula is inserted under the skin using a needle, which is removed leaving only the flexible tube under the skin. The pump user replaces the cannula every 2-3 days.
What is a smart pen?
A smart pen is an insulin pen that does more than just deliver insulin. Smart pens communicate with special smartphone apps that help you to calculate your insulin doses and track your insulin delivery, food intake, and glucose readings.
Technology Shared Medical Appointments
Insulin pump virtual SMA
A virtual shared medical appointment (SMA) led by the endocrinology pharmacist. The single-session 1.5 hour class includes a presentation on available diabetes technology options, including: continuous glucose monitors, connected insulin ‘smart’ pens, and insulin pumps. The class provided an opportunity for patients interested in these technologies to ask questions in a real-time, open-discussion format with other patients.
Continuous glucose monitor SMA
The continuous glucose monitor (CGM) shared medical appointment (SMA) is a two-part appointment that takes place on consecutive Tuesdays.
Part one of the CGM SMA is a 1-hour appointment in a classroom format. This includes a short presentation providing a basic overview of CGM and specifics on the device that the patients will be wearing. Following the presentation the CGM device will be inserted.
Part two of the CGM SMA is a 1.5-hour class where the CGM data from the previous week is downloaded and reviewed by an endocrine clinical pharmacist. This is done in an SMA format which allows for shared discussion and support amongst patients with diabetes. Patients referred to the class consent to the collaborative practice agreement shared between their endocrinologist and the endocrine clinical pharmacist, allowing for medication therapy adjustments based off downloaded CGM data.

Comprehensive Diabetes Clinic
Our Comprehensive Diabetes Clinic offers a convenient program that includes meeting with the following specialists during your individualized 2 to 2 1/2 hour session:
- An endocrinologist specializing in diabetes.
- A pharmacist to clarify any remaining concerns about your diabetes medications and to set up your continuous glucose monitor (CGM) follow-up visit.
- A Diabetes Care & Education Specialist (DCES) to help patients achieve their diabetes-related goals through comprehensive diabetes education and support.
You will have the option of having a CGM (such as Dexcom or Freestyle Libre) inserted. A follow-up visit (virtual or in-person) will be arranged at 2 weeks to download your glucose readings and give your recommendations on how to make adjustments to your meals, physical activity, and diabetes medications. There is optional exercise training and e-coaching with an endocrinology exercise physiologist.
