Goal: We will discuss topics important to the management of diabetes. These will include a discussion of diet and exercise, medications for diabetes along with goal-setting for weight loss and management of hypertension and cholesterol. We will also address preventive care to avoid diabetic complications. We cover the standards of care. We refer to it as ABCDEF: review A1C levels, blood pressure, cholesterol, diet, eye care, exercise and feet issues.

Dr. Fernandez is an internationally recognized expert in movement disorders who has been voted one of the Best Doctors in America by his peers. After completing his medical training, he joined the faculty of Brown University School of Medicine as Assistant Professor of the Department of Clinical Neurosciences and served as Associate Director of the Movement Disorders Unit and Neurological Director of its Functional Neurosurgical Program. In 2003, Dr. Fernandez relocated to the University of Florida, where he eventually became Director of the Clinical Research Unit for Neurological and Psychiatric Disorders, Vice Chair of Academic Affairs, and Professor of Neurology prior to joining Cleveland Clinic. An active and productive researcher, he has initiated or participated in over 50 clinical trials and has published his findings in well over 300 articles and abstracts on Parkinson’s disease, cervical dystonia, blepharospasm, and other movement disorders. He has nearly 40 published book chapters and books to his credit, and has served on the editorial board of Movement Disorders and is currently an editorial board member of the American Journal of Clinical Neurology, European Neurological Journal, and Clinical Neuropharmacology.

He uses traditional conservative medical treatments and incorporates osteopathic spinal manipulation when appropriate. He is a member of the American Osteopathic Board of Family Physicians and is certified in osteopathic manipulation by the American Osteopathic Board of Neuromusculoskeletal Medicine.

This is a system of evaluation and treatment designed to achieve and maintain health by restoring normal function to the body by balancing body structure with manipulation. Manipulation is the therapeutic application of manual pressure or force. Oftentimes, structural problems in the spinal column can affect the nerves that radiate out to the various organs, thus contributing to disease.

OMT is typically used to treat musculoskeletal disorders, such as low back pain, neck pain, pelvic pain, sports injuries, repetitive stress injuries, and tension headaches. While osteopathic and chiropractic techniques overlap, they are not identical. As a general rule, chiropractic manipulation uses direct thrust techniques on the spine (HVLA), while osteopathic practitioners - such as Dr. Wilson - use other, gentler techniques, as well as HVLA.

Title: Stroke Survivors

Basic Information

During a Shared Medical Appointment for Stroke survivors, stroke survivors can learn from each other various ways to gain control of their personal risk factors while having the benefit of a health care professional present to provide expertise on the information discussed.

Using guidelines from the American Stroke Association, the focus of the discussion in the SMA is on achieving optimal stroke risk factor control. Creative ways on how to handle the following controllable risk factors are shared and discussed, high blood pressure, high cholesterol, diabetes, being overweight or obese, diet, smoking, a sedentary lifestyle and alcohol intake.

In addition, each patient in the shared medical appointment takes part in an individualized neurological assessment by an advanced practice nurse (nurse practitioner). This is done in a private exam room and addresses individual recovery, symptoms or concerns.

This SMA consists of adult patients with diabetes who have been seen by a nurse practitioner and requires a follow up visit. This is run under the guidance of Leighanne Hustak, CNP and facilitators Gretchen Petz and Lauren Sullivan. This group will address education and improved self-management for patients with diabetes. In many cases, you can control diabetes through better nutrition, a healthy weight, physical activity, and regular checkups with your health care team.

Sometimes medication is also necessary, which your health care provider will determine. Medication, if prescribed by your doctor or nurse practitioner, is a crucial component of diabetes management and should be taken as directed. Some medications need to be timed with meals, and if so your doctor or nurse practitioner will instruct you on appropriate timing. What, when and how much you eat are all important factors in managing diabetes. With the help of your health care professional, you should develop and follow a meal plan based on your individual needs.

The appointment includes a discussion of incidence and risks of skin cancer development as well as prevention and management of skin cancers in high risk patients. Group discussion, individual question and answer session as well as a complete skin examination are included in these appointments.

Alexandra Zhang, MD is a board certified dermatologist. She completed her fellowship at Cleveland Clinic in dermatologic surgery and cutaneous oncology. She has special interest in organ transplant for skin cancers.

Brief Description
These 60-90 minutes Shared Medical Appointments (SMA’s) are for organ transplant candidates and organ transplant recipients who are at high risk of developing multiple aggressive skin cancers. She proceeded to do a research fellowship at the Bone Mineral Research Center at Winthrop University Hospital in New York, doing research on osteoporosis, vitamin D and bone metabolism. Dr. Li-Ng then completed her endocrinology fellowship at Georgetown University Hospital and Washington Hospital Center in Washington, D.C. She currently serves as the Associate Program Director of the endocrinology fellowship program.

Description

Goal: This Shared Medical Appointment (SMA) for people (both male and female) with multiple sclerosis. The goal of this SMA is to educate patients regarding a new oral Multiple Sclerosis (MS) medication named Gilenya. Gilenya is an oral disease modifying treatment for MS. Gilenya was approved to reduce relapses and disability progression in patients with relapsing forms of MS. For patients with ongoing disease activity, intolerable side effects, or trouble with the previously-available MS therapies, Gilenya is an appropriate option. After being administered Gilenya, patients are monitored for 6 hours after their first dose and are observed for any potential side effects or allergic reactions.

This 90-minute SMA is for pediatric and adolescents patients with PCOS. Each SMA will cover a different subject, covering PCOS topics and healthy lifestyle to help patients make positive change at a time. There will be a Nutritionist at every SMA and in some sessions a behavior specialist or sports medicine specialist.

Polycystic ovarian syndrome (PCOS) is among the most frequent causes of infertility in adult women. PCOS is also the most common cause of menstrual dysfunction and hyperandrogenism in adolescents. It is challenging, yet important, for pediatricians to identify and treat this condition when it presents in childhood and adolescence. Early identification and treatment of PCOS will help prevent long-term consequences, which in addition to infertility, include type 2 diabetes, endometrial hyperplasia and increased risk of carcinoma.

Nauhad Raissouni, MD is a board-certified pediatrician who practices within the Center for Pediatric Endocrinology. She completed her pediatric endocrinology fellowship at SUNY Downstate Medical Center in Brooklyn, New York.

Christine Bowen is a licensed dietician. A registered, licensed dietitian practices the science of medical nutrition therapy. This shared medical appointment is for new patients that have been recently diagnosed as having osteoporosis, osteopenia, bone loss or who has recently had a metabolic bond consult. This SMA occurs on the last Friday of each month from 2 p.m. - 3:30 p.m.

Osteoporosis is a disease that weakens bones, making them more susceptible to sudden and unexpected fractures. Literally meaning "porous bone," it results in an increased loss of bone mass and strength. The disease often progresses without any symptoms or pain. Generally, it is not discovered until weakened bones cause painful fractures. Most of these are fractures of the hip, wrist, and spine.

Although osteoporosis occurs in both men and women, women are four times more likely to develop the disease than men. Thankfully, there are steps you can take to prevent osteoporosis from ever occurring. Treatments can also slow the rate of bone loss if osteoporosis is present. After age 50, one in two white women, and one in four white men will have an osteoporosis-related fracture in their lifetimes. An additional 30 percent have low bone density that puts them at risk of developing osteoporosis (including African Americans). Osteoporosis is responsible for more than 2 million fractures each year, and this number continues to grow.

The Minority Men’s Health Center’s “Minority Men’s Wellness” Shared Medical Appointment Clinics are opportunities for male patients to meet with a Urologist (Dr. Modlin) and other health providers of the Minority Men’s Health Center in a group setting with 5-10 other patients. Our SMA’s offer health information for men on a variety of topics including prostate disease, prostate cancer, erectile dysfunction, hypertension, diabetes, kidney disease, nutrition, preventive health, and wellness. The advantage of the SMA is that attendees are able to hear, interact with and learn from other patients about their particular medical concerns, which they may in turn also be experiencing. The Chronic Kidney Disease (CKD) shared medical appointment consists of 8-12 patients sharing the common medical diagnosis of CKD (decreased glomerular filtration rate < 60="" ml/min)="" whether="" it="" be="" in="" the="" setting="" of="">diabetes, high blood pressure or any other cause. It will entail review of medications, blood pressure and all pertinent labs related to their disease.

Brief Description
Kidney diseases occur when the nephrons are damaged and cannot filter the blood. The damage can happen quickly, such as when it is caused by injury or toxins. However, most damage occurs over a period of time. High blood pressure (hypertension) and diabetes are the two most common causes of the progressive type of damage to the nephrons and the resulting kidney failure (chronic kidney disease or CKD). CKD is also known as chronic renal insufficiency.

Who is at risk for chronic kidney disease?

  • People with diabetes
  • People with high blood pressure
  • People with a family history of kidney disease
  • People who often use painkillers, including over-the-counter products such as aspirin and ibuprofen

Brief Description
To increase accessibility for patients, and to address the concerns over the potential out-of-pocket costs of this treatment. This method allows a group of patients to receive treatment at the same time in one room. Patients remain fully clothed, and simply roll up their pant and shirt sleeves while sitting in recliner chairs. A one-hour initial consultation and private acupuncture treatment is required prior to the Acupuncture SMA. Cleveland Clinic’s Children's Health Essentials Moms program will give you the information you need to optimize your weight and health before and during pregnancy, and after delivery. Too often, women who are significantly overweight feel there is nothing they can do about it – they’ve tried everything. Our goal is to educate and empower you on what a healthy weight for you is. We do this through a 60 minute shared medical appointment with a nurse midwife and registered dietician who have expertise in weight management.

Brief Description
The goal of this Shared Medical Appointment is to provide you with information to optimize your weight and health before and during pregnancy and after delivery. A nurse midwife and a registered dietician will lead the group in a 1 hour session at various Cleveland Clinic locations, including Independence Family Health Center and the Columbia Road Medical Center in Westlake.
