Our longitudinal care clinic experience is achieved through the Internal Medicine continuity clinic at different locations throughout the western region of Cleveland.
Our schedule follows an X + Y (8 + 2) model. Residents will rotate for eight weeks on different rotations, followed by two weeks of outpatient clinic.
Over the three years, residents are tasked with an increasing number of patients and responsibilities. The Westown Health Center, Independence, and Cleveland Clinic Lakewood - Medical Outpatient Center ensure continuity of care by scheduling follow-up appointments for the same patients with their assigned residents. Our extensive primary care experience includes providing care to a diverse range of local communities, particularly the underserved population.
- The residents will discuss with their preceptor each new and old patient they see in the longitudinal care clinic.
- The preceptor will review pertinent historical information and/or physical examination findings with the resident and the patient.
- The preceptor is required to sign on all patients seen by the resident, certifying his concurrence with the plan of diagnostic and therapeutic intervention by the resident.
- Preceptors evaluate the residents quarterly.
- The residents also evaluate the preceptors.
