Goal: The goal of this SMA is to help manage patients’ recovery to addiction. This SMA is for people who have an addiction to opiates and are established patients of Dr. Robert Bale. Patients of Dr. Bales who are on a stable dose of Suboxone, should attend this SMA monthly. Suboxone is used to treat narcotic (opiate) addiction. Together with a doctor who understands opioid dependence, SUBOXONE can help you manage your physical symptoms and cravings, allowing you to work toward your recovery. Each month there will be a different topic of interest related to comprehensive addiction therapy.

Center for Functional Medicine

Description
Center for Functional Medicine (CFM) SMAs are an extension of the medical office visit, and is conducted by Functional Medicine physicians with a Health Coach as the facilitator. The appointment is exactly 90 minutes and held in the company of up to 15 other patients who would also like to benefit from having extra time with their healthcare providers. The appointment begins and ends on time and there is no time spent in the waiting room. During the appointment patients will have time to share details of their history and symptoms, explore the Functional Medicine Matrix, and activate an elimination/reset diet to begin a life of wellness.

Patients will participate in a group discussion setting in which they will share and learn from each other while being supported by the Center for Functional Medicine team. The appointments consist of 4 sessions that take place every other week at Chagrin Falls Family Health Center, 1:00 p.m. on Tuesdays OR 3:00 p.m. on Thursdays.

This appointment format provides more time for provider and group interaction. Center for Functional Medicine SMA's will:

Chronic Heart Failure

Goal: This SMA is for heart failure patients’ first visit to the Congestive Heart Failure Center and for those heart failure patients who were recently discharged from Cleveland Clinic. A family member is encouraged to attend this important appointment. The appointment includes an assessment of the patients’ Chronic Heart Failure (CHF); review of medications, their side effects, and taking as prescribed; discussion of nutritional needs and how to read food labels; discussion of what is heart failure, daily weights, signs and symptoms; smoking and alcohol cessation. Description
Goal: This SMA is for adult patients with diabetes who require a follow up visit. This is run under the guidance of Leighanne Hustak, CNP. This appointment will address education and improved self-management for patients with diabetes. In many cases, you can control diabetes through better nutrition, a healthy weight, physical activity, and regular checkups with your health care team.

Sometimes medication is also necessary, which your health care provider will determine. Medication, if prescribed by your doctor or nurse practitioner, is a crucial component of diabetes management and should be taken as directed. Some medications need to be timed with meals, and if so your doctor or nurse practitioner will instruct you on appropriate timing. What, when and how much you eat are all important factors in managing diabetes. With the help of your health care professional, you should develop and follow a meal plan based on your individual needs.

Dizziness

Goal: This SMA is a first visit appointment for patients with symptoms of dizziness and imbalance to the Head and Neck Institute. During the SMA, the providers will perform an evaluation and make appropriate management recommendations. Patients will also receive counseling and educational resources pertaining to their dizziness-related conditions. Multiple Sclerosis

Title: Gilenya First Dose Education Session

Description
Goal: This Shared Medical Appointment (SMA) for people (both male and female) with multiple sclerosis. The goal of this SMA is to educate patients regarding a new oral Multiple Sclerosis (MS) medication named Gilenya. Gilenya is an oral disease modifying treatment for MS. Gilenya was approved to reduce relapses and disability progression in patients with relapsing forms of MS. For patients with ongoing disease activity, intolerable side effects, or trouble with the previously-available MS therapies, Gilenya is an appropriate option. After being administered Gilenya, patients are monitored for 6 hours after their first dose and are observed for any potential side effects or allergic reactions.

Osteoporosis

Goal: This Shared Medical Appointment (SMA) is for women with osteoporosis. The goal of this SMA is to educate those who have been recently diagnosed as having osteoporosis, osteopenia, bone loss or who recently had a metabolic bone consult. Dr. Al-Ashkar conducts this SMA with certified adult nurse practitioner Beth Wilson.

Osteoporosis is a disease that weakens bones, making them more susceptible to sudden and unexpected fractures. Literally meaning "porous bone," it results in an increased loss of bone mass and strength. The disease often progresses without any symptoms or pain. Generally, it is not discovered until weakened bones cause painful fractures. Most of these are fractures of the hip, wrist, and spine.

Although osteoporosis occurs in both men and women, women are four times more likely to develop the disease than men. Thankfully, there are steps you can take to prevent osteoporosis from ever occurring. Treatments can also slow the rate of bone loss if osteoporosis is present. After age 50, one in two white women, and one in four white men will have an osteoporosis-related fracture in their lifetimes. An additional 30 percent have low bone density that puts them at risk of developing osteoporosis (including African Americans). Osteoporosis is responsible for more than 2 million fractures each year, and this number continues to grow.

Title: Taking Control of Parkinson's Disease

Goal: We will discuss topics important to the management of diabetes. These will include a discussion of diet and exercise, medications for diabetes along with goal-setting for weight loss and management of hypertension and cholesterol. We will also address preventive care to avoid diabetic complications. We cover the standards of care. We refer to it as ABCDEF: review A1C levels, blood pressure, cholesterol, diet, eye care, exercise and feet issues.

Dr. Fernandez is an internationally recognized expert in movement disorders who has been voted one of the Best Doctors in America by his peers. After completing his medical training, he joined the faculty of Brown University School of Medicine as Assistant Professor of the Department of Clinical Neurosciences and served as Associate Director of the Movement Disorders Unit and Neurological Director of its Functional Neurosurgical Program. In 2003, Dr Fernandez relocated to the University of Florida, where he eventually became Director of the Clinical Research Unit for Neurological and Psychiatric Disorders, Vice Chair of Academic Affairs, and Professor of Neurology prior to joining Cleveland Clinic. An active and productive researcher, he has initiated or participated in over 50 clinical trials and has published his findings in well over 300 articles and abstracts on Parkinson’s disease, cervical dystonia, blepharospasm, and other movement disorders. He has nearly 40 published book chapters and books to his credit, and has served on the editorial board of Movement Disorders and is currently an editorial board member of the American Journal of Clinical Neurology, European Neurological Journal, and Clinical Neuropharmacology.

Goal: This Shared Medical Appointment is an innovative and interactive approach for your annual physical. This is a health and wellness focused appointment with private physical exam. You will join up to 5 other women in a setting that encourages asking questions, learning from one another by sharing experiences, and promoting a healthy lifestyle. Your individual questions, concerns, and medication refills are addressed during this SMA.

Provider Information
Pelin Batur, MD is a board-certified internist at Cleveland Clinic. She graduated from medical school at Northeastern Ohio Universities College of Medicine, where she completed the accelerated, 6-year BS/MD track. She completed her internal medicine residency training at Cleveland Clinic. With the goal of providing more comprehensive care in all aspects of women’s healthcare, she then completed specialty fellowship training in women’s health at Cleveland Clinic. Her practice now consists of seeing women’s health consults as well as a general internal medicine patients (both men and women).

Her areas of expertise include contraception (especially difficulties with multiple hormonal options), abnormal bleeding (pre or post menopause), breast concerns (including options for high risk patients), osteoporosis, headache, polycystic ovaries, menopause (discussing all types of hormone therapy). She also performs specialty procedures including colposcopy (for evaluation of abnormal pap smears), IUD insertion (contraceptive), endometrial biopsies (for evaluation of heavy menstrual bleeding), cervical polyp removal, vulvar biopsies, and ductal lavage (a procedure for women at high risk for breast cancer).

Dr. Batur runs them with Registered Nurse Lori Sherwood. Lori serves as the behavioral health specialist and facilitator.

Archana Gorty, MD came to Cleveland Clinic in 2010 after she completed her fellowship in Preventive Medicine at the Mayo Clinic. She is board certified in internal medicine and public health and general preventive medicine.

Patient Population
All established patients of Dr. Gorty who are English speaking, hearing adults who are ambulatory and in possession of their faculties and do not have a highly contagious disease at the time of the group appointment.

Lilian Gonsalves, MD, is Clinical Professor of Medicine, Cleveland Clinic Lerner College of Medicine, and Staff, Department of Psychiatry and Psychology. She holds a joint appointment in the Department of Obstetrics and Gynecology and the Gault Women’s Health Center. She is a Cleveland Clinic-trained, board-certified psychiatrist with interest and expertise in mood disorders in women, chronic pain and psychological aspects of the medically and surgically ill patient. She has also successfully used the group model for medication management in women with anxiety and mood disorders.

Patient Population
Women 18 and over with mood and anxiety disorders. We exclude severe personality disorders, psychotic and demented patients.

Description
Goal: This Shared Medical Appointment addresses medical issues of women related to menopause, osteoporosis, hormone therapy, sexual health, and any other female health concerns. Patients are seen individually in the context of a shared group setting with the attending physician, nurse and ancillary health care members. Patients often schedule bone density and mammograms on the same day. These tests are done within our facility. Patients in the Physical/Annual Exam SMA also are examined in a separate exam room first. Shared visits are not appropriate for urgent medical problems.

Holly L. Thacker MD, FACP, director of the Cleveland Clinic Center for Specialized Women’s Health and Associate Professor, Cleveland Clinic Lerner College of Medicine of Case Western Reserve University, recently published her second women’s health book “Cleveland Clinic Guide to Menopause.” At the end of 2008, the Cleveland Clinic Women’s Health Institute was gifted with the National Speaking of Women’s Health and Dr. Thacker now serves as the Executive Director.

She was honored by the American Medical Women’s Association (AMWA) and the Journal of Women’s Health with the "Lila Wallis Women’s Health Award" in recognition of Dr. Thacker’s lifetime achievements, accomplishments, motivation, mentorship, energy and enthusiasm for women’s health, education, and research during the AMWA annual meeting and Women’s Health 2009: The 17th Annual Congress, in Williamsburg, VA.

Women's Health - Pregnancy

Title: Back Pain in Pregnancy

Goal: This goal of this SMA is to reduce back pain and pelvic pain during pregnancy with the help of the partner / spouse. This SMA offers women and their partners medical advice and education for women who are suffering from back pain and pelvic pain related to their pregnancy. Both must attend the appointment for this to be effective. Dr. Frederick Wilson, along with physical therapist Barb Domsic PT, will work with patients that have been referred by their Obstetrics and Gynecology provider.

Discussions will include the topic of osteopathic manipulative therapy (OMT). This is a system of evaluation and treatment designed to achieve and maintain health by restoring normal function to the body by balancing body structure with manipulation. Manipulation is the therapeutic application of manual pressure or force. Oftentimes, structural problems in the spinal column can affect the nerves that radiate out to the various organs, thus contributing to disease. OMT is typically used to treat musculoskeletal disorders, such as low back pain, neck pain, pelvic pain, sports injuries, repetitive stress injuries, and tension headaches. While osteopathic and chiropractic techniques overlap, they are not identical. As a general rule, chiropractic manipulation uses direct thrust techniques on the spine (HVLA), while osteopathic practitioners - such as Dr. Wilson - use other, gentler techniques, as well as HVLA.

Provider Information
Fredrick Wilson, DO is an osteopathic medical specialist who treats patients with back and neck pain. He uses traditional conservative medical treatments and incorporates osteopathic spinal manipulation when appropriate. He has been on staff at Cleveland Clinic since 1997 and has been the Director of the Solon Cleveland Clinic Center for Spine Health since 2000. He is a member of the American Osteopathic Board of Family Physicians and is certified in osteopathic manipulation by the American Osteopathic Board of Neuromusculoskeletal Medicine.

Patient Population
All established English speaking women who are pregnant and have back and pelvic pain; who are in possession of their faculties and do not have a highly contagious disease at the time of the group appointment.
