Ralph Waller was helping a friend with farm equipment in Georgia when something unexpected happened. As Ralph swung a sledgehammer, his friend noticed that he was becoming unusually short of breath. It was the first sign that something might be wrong with his heart.
Ralph sought care near his home, where a cardiac MRI revealed aortic valve regurgitation, also called aortic insufficiency. The condition occurs when the heart’s aortic valve does not close completely, allowing blood to leak backward into the heart instead of flowing normally to the body. Over time, the heart must work harder to circulate blood, which can cause shortness of breath, fatigue and other symptoms.
With a diagnosis in hand, Ralph turned to a healthcare system he already trusted. In 2009, he had undergone mitral valve surgery at Cleveland Clinic in Ohio and remembered the positive experience. This time, his search for answers led him to Cleveland Clinic Indian River Hospital in Vero Beach, Florida.
Ralph and his wife, Kendall, had purchased a travel trailer with plans to visit clients throughout Georgia and Florida. They never expected it would become part of a journey to restore Ralph’s health.
After learning about his heart condition, the couple traveled to Vero Beach to meet with James Wudel, MD, Director of the Heart, Vascular and Thoracic Institute at Cleveland Clinic Indian River Hospital and the Dr. Cary L. Stowe Endowed Chair for Cardiovascular Surgery.
“When you come down here and experience this, you’ve got to tell somebody,” Ralph says. “It’s like a breath of fresh air.”
As the team evaluated Ralph, they discovered that his aortic valve was not his only concern. Imaging also revealed an ascending aortic aneurysm, a dangerous bulging and weakening in the portion of the body’s main artery closest to the heart. If an aneurysm continues to enlarge, it can tear or rupture and cause life-threatening bleeding.
Richard Moore, MD, a Cleveland Clinic cardiologist, confirmed that Ralph’s aortic valve regurgitation was severe. Because Ralph was experiencing symptoms and also had an ascending aortic aneurysm, the team recommended surgery.
“Mr. Waller came to Vero Beach for evaluation of his regurgitant aortic valve,” Dr. Wudel says. “Dr. Moore confirmed the severity of the regurgitation and recommended surgery based on Mr. Waller’s symptoms. We also found an ascending aortic aneurysm that needed to be repaired at the time of his valve surgery.”
Dr. Wudel took time to explain Ralph’s conditions, show the couple what needed to be repaired and walk them through the proposed procedure.
Ralph underwent open-heart surgery to replace his damaged aortic valve and replace the weakened, enlarged portion of his ascending aorta. Addressing both conditions during the same operation helped restore proper forward blood flow through the heart while reducing the risk that the aneurysm could continue to enlarge, tear or rupture.
Replacing the damaged valve prevents blood from leaking backward into the heart with each heartbeat. This reduces the heart’s workload and allows it to pump blood more efficiently. For patients such as Ralph, that can lead to noticeable improvements in breathing, stamina and the ability to perform everyday activities.
Ralph and Kendall stayed nearby in their travel trailer before and after the operation so they could remain close to the care team they had come to trust. What began as a frightening and uncertain chapter gradually became one marked by reassurance and confidence.

Ralph and Kendall arrived in Vero Beach carrying more than concern about his heart. Difficult experiences during Kendall’s previous medical care had made them cautious and especially aware of the importance of compassionate, coordinated care.
At Cleveland Clinic Indian River Hospital, they found a different experience. Beginning with the first phone call, team members helped coordinate Ralph’s medical records, appointments and next steps. That support helped the couple feel they were no longer navigating his condition alone.
Before deciding on surgery, Ralph wanted honest guidance. He asked a caregiver what she would do if someone she loved needed heart surgery. Her response gave him peace and reinforced the trust he was developing in his surgeon and the care team.
“If my loved one needed heart surgery, it would be with Dr. Wudel,” Ralph recalls her saying.
That trust continued to grow throughout his hospitalization. From diagnostic testing to the surgical intensive care and step-down units, Ralph encountered caregivers who were both highly skilled and attentive to his needs. Nurses reassured him during difficult moments, helped him begin moving after surgery and encouraged him as he worked on breathing exercises and walking.
Ralph was also struck by the teamwork, cleanliness and genuine concern he observed throughout the hospital.
“You are an example of what medical care should be,” he says. “Every hospital should be like this. You have set the bar so high.”
Ralph noticed a difference soon after surgery. As he recovered, his breathing and stamina improved, he experienced little pain and he regained energy he had not realized he was missing. He described the feeling as “euphoric.”
“With his new heart valve, Mr. Waller noticed an immediate improvement in his stamina and breathing while he was still in the hospital,” Dr. Wudel says. “Those improvements continued during his recovery as he became better able to perform his daily activities.”
For Ralph, the experience restored more than his physical health. It renewed his trust and optimism. He and Kendall spent time exploring Vero Beach and meeting people at church, in restaurants and throughout the community. What began as a trip for heart surgery also became a journey filled with connection and encouragement.
Ralph plans to continue receiving follow-up care from Cleveland Clinic physicians, even though doing so requires traveling several hours from his Georgia home. For him, the decision is simple: his health is worth the journey.
