Hospital Rounding with Dr. Richard Wardrop
Podcast Transcript
Today, we have a wonderful episode planned and the topic is Hospital Rounding
My name is Andrei Brateanu and I am one of the APDs here at CCF IMRP.
I am delighted to introduce today’s podcast, which features none other than Dr. Wardrop as our esteemed guest and clinical expert. This special episode promises to be truly exceptional, thanks to Dr. Wardrop’s invaluable insights and expertise. He is a Med-Peds clinician educator, ABIM Chair for the Internal Medicine Board.
We are also honored to have with us Dr. Stavros Stefanopoulos and Dr. Christina Hermanns, both members of our PGY-3 class planning to go into hospital medicine.
To our honored guests, please take a moment to say hi, tell us about yourselves, and then we will start with the questions.
Part 1: Rounding Physiology
Question 1 (Dr. B): Dr Wardrop, can you tell us a little bit about the history of rounding?
Question 2 (Stavros): There are a lot of personal preferences towards various rounding approaches: table versus bedside rounds for example, but what data or literature is there about which one is better for patients?
Question 3 (Christina): How about for providers? Any context for which workflow is more efficient?
Question 4 (Stavros): What are some other higher value rounding strategies, including maybe plan of care rounds?
Part 2: Low Value Care / Things we do for no reason
Question 5 (Dr. B): TWDFNR has become a staple of our IMRP topics, you have a personal connection to its inception right Dr. Wardrop?
Question 6 (Christina): What are some things that we do on rounds that are potentially low value?
Question 7 (Stavros): What are some examples of known TWDFNR?
Question 8 (Christina): Let’s talk a little bit more about sleep management in the hospital? Can we dive deeper into how we should go about changing some of these low yield practices?
Question 9 (Dr. B): What about being NPO after midnight? Any thoughts on how this practice could be made better?
Part 3: Opportunities we don’t embrace on rounds
Question 10 (Stavros): Thank you Dr. Wardrop, with these ideas in mind, what are some opportunities for us to do a better job of on rounds?
Question 11 (Christina): Another common issue facing hospitalists across the country are some strategies to implement better ways of treating substance misuse at discharge?
Question 12 (Dr. B): How should we approach more specialized care situations that feel different from the norm?
Part 4: Summary of recommendations
Christina: So let’s summarize what all trainees and really anyone who rounds in a hospital setting can take away from this.
Stavros: Dr. Wardrop could you give us a one liner on what your biggest clinical pearl for successful rounds?
Dr. B: Thank Dr. Wardrop and to our phenomenal senior residents for this wonderful discussion. We hope this will be useful to all the young physicians planning to go into hospital medicine and also to existing hospitalists that listened to our podcast.
