Ordering Supplements
Step 1. Visit the Healthy Living Shop
Step 2. Get started: create your account
- Enter your email address which will be your username
- Create password
- Medical Record Number (MRN) is listed on your AVS
- Select your provider from the dropdown box. If they are not listed, select “other”
- Enter Provider Code: functional (it is not case specific)
Step 3. Order supplements
- Enter the supplement name in the search box
- Add all supplements to your cart and proceed to checkout. Orders of $100 or more qualify for free shipping.
