Overview
The CRT-HF Follow-Up Clinic is a specialized, one-time, multidisciplinary clinic designed for all patients who received a CRT pacemaker or defibrillator. An appointment will be made for you to be seen approximately 6-months after device implantation. During your visit you will meet with a team of clinicians who will evaluate to what extent your heart has benefited from the device, and if there is a possibility to further improve your cardiac medications or device settings.
As part of this comprehensive evaluation you will undergo an echocardiogram, functional testing with a nurse, and a complete medical evaluation by both a cardiac electrophysiologist and a congestive heart failure specialist. Your plan of care will be determined based on your needs, and will be communicated to your regular cardiologist with whom you will continue your care after this visit. All appointments related to CRT-HF Follow-Up Clinic are held at the Miller Pavilion, at Cleveland Clinic’s Main Campus.
If you have questions regarding your appointment contact CRTHF@ccf.org or 216.444.4372.
