Community Care Outcomes

Community Care now reports rates of adolescent well-care visits for patients aged 12 to 17. The previous population of adolescent well-care visits included patients aged 12 to 21, which accounts for the lower "N" size. Cleveland Clinic's Wellness Registry allows care teams to identify patients who are overdue for visits so that caregivers can reach out to patients to assist in scheduling. Community Care is particularly focused on improving vaccine compliance through well-care visits. No Cleveland Clinic target is available for 2022.
