ACO 22: Blood Sugar Control in Patients With Diabetes With Hemoglobin A1c Less Than 8 Percent

(HbA1c < 8%)

Community Care physicians closely monitor the care of patients with diabetes. Optimizing blood sugar control helps patients lower their risk of serious complications of diabetes such as heart attacks and strokes. The percentage of patients with diabetes aged 18 to 75 whose blood sugar is adequately controlled (HbA1c < 8%) is reported, as well as the percentage of patients with poor control (HbA1c ≥ 9%).
