Overview

Our group characterizes sleep disordered breathing and its intersection with pulmonary disease and other related diseases (e.g., metabolic abnormalities) associated with 1) nocturnal or daytime hypoventilation, including chronic obstructive pulmonary disease, neuromuscular disease, and the obesity hypoventilation syndrome, and 2) pulmonary hypertension. We are leveraging our sleep study registry to examine sleep related hypoventilation and obesity associated sleep hypoventilation and adverse outcomes post-operatively in bariatric surgery. Our interest is more specifically on the pathophysiology of hypoventilation, associated sleep-disordered breathing, modeling of the course of lung function, and objective measures of the impact of non-invasive ventilation, as well as pitfalls of ventilation including patient-device asynchronies.

As the lead of the Sleep Core for the multicenter Pulmonary Vascular Disease Phenomics Program (PVDOMICS) NHLBI-funded study, an in-progress observational study designed to classify pulmonary hypertension by molecular, physiologic, and imaging biomarkers, our group is also focused on the investigation of sleep disordered breathing (SDB) and pulmonary hypertension inter-relationships. Therefore, we are poised to examine various aims including the prevalence of SDB subtype (i.e., obstructive versus central sleep apnea/periodic breathing) and degree. In addition, we are examining the patterning of hypoxia across the phenotypic clusters of pulmonary hypertension including established World Symposium Pulmonary Hypertension (WSPH) categories, as well as newly identified phenotypes characterized by this study, and associations with cardiac substrate and invasive cardiopulmonary physiologic indices.
