Self-Medication Guidelines

We believe it is important for you to be an active participant in your health care. The first step in caring for your health is learning how to take your medications.

Our Self-Medication Program will help you learn about your medications, including the purpose, dose, frequency and possible side effects, while you are still in the hospital.

Developing a medication routine while you are in the hospital helps make it easier for you to follow your medication schedule when you go home.

We know that learning about taking your medications can be overwhelming at first. But remember, you will learn a little each day.

You will follow the steps below and gradually progress to taking your medications by yourself after you have demonstrated certain skills, and when both you and your nursing team feel comfortable with your progress. Always discuss your concerns with your health care team.

Steps for taking your medications in the hospital

  • You will receive a list of your medications. You will be provided with information about each medication and a medication record that states when the medication should be taken. You will need to know the name and purpose of each medication, as well as the prescribed dose and frequency. Your nurse will bring your medications to your room. If you have any questions about your medications, please ask your nurse.
  • You will select the right medication(s) to be taken as directed at the right time(s). You will get the medications ready to take.
  • Your nurse will check the medications you have prepared and give approval for you to take your medications. Your nurse will also ask you questions to be sure you know why you are taking the medications, what to expect, and the possible side effects associated with the medication.
  • After taking your medication, complete the medication record at your bedside as instructed (write the time you took the medication in the designated space).
  • Store your medications according to your nurse’s instructions in the locked box that was provided to you. The key to the box must remain with you at all times.
  • When your nurse determines that you are able, you will take your medications without your nurse’s approval. You must take your medication(s) within 30 minutes of the scheduled time. For example, if you are supposed to take your medication at 10:00 am, you must take it between 9:30 am and 10:30 am.
  • Your nurse will check your medication record throughout the day. You will be notified of any changes to your medications, such as time or dosage.
  • If a medication is discontinued, the unused medication will be returned to the hospital pharmacy.
