We want to help you stay healthy after you leave the hospital. Your healthcare team will make sure you get the right medications, follow-up care and information to manage your heart failure. Your job is to take the medications exactly as directed, keep all of your appointments and make the needed changes to your diet and routine. We understand that all of this can be a lot to do, but it is the only way to keep your heart failure from getting worse.
