Thirty-Day All-Cause Readmission Rate
Community Care's hospitalist provides inpatient care for a wide range of diagnoses and conditions. To better determine readmission rates, leadership continues to focus on the discharge status of patients and to pair interventions with each of the main dispositions to which patients are discharged. The three main discharge dispositions are home, home with home healthcare, and skilled nursing facility. Care coordination efforts contribute to decreased readmission rates for all three dispositions.
