Daniel (Dan) Blaz has 18 years’ experience as a Certified Nurse Practitioner – focusing in Family Medicine. He has been a board certified Family Nurse Practitioner since 2006. Dan joined the Cleveland Clinic enterprise in 2019 working in Family Medicine, prior to this he served over 21 years in the U.S. Army Medical Department. In 2022 Dan became the Director of Advanced Practice Providers for the Urology and Kidney Medicine at the Cleveland Clinic Main Campus. Dan's clinical practice is nested with Urology where he focuses on men's health conditions.
Dan began his career in the military as a University of Akron Army ROTC Scholarship Recipient. This led to over 21 years of active duty service and a myriad of clinical, operational and leadership positions. Dan’s last leadership position in the U.S. Army was that of Chief of Clinical Operations at Army Central Command (Shaw Air Force Base, SC), where he was responsible for Army clinical operations in 21 countries in the Middle East. Other clinical, operational and key leader roles during his military career in the Army Medical Department included: Medical Director of an Army Medical Home Clinic, Deputy Program Director of the FNP Program at the Uniformed Services University Graduate School of Nursing, Assistant Deputy at the Army Physical Fitness Research Institute and Field Surgeon 520th Medical Company (Area Support) – Iraq.
Dan obtained his Doctor of Nursing Practice at the University of Missouri-Kansas City in 2015 and has a robust academic and teaching background. He has a research concentration in military medicine, transition to practice, men’s health, Point of Care US, breast cancer, and wellness/fitness. Dan has presented on these topics at the regional and national levels. He is a regular reviewer for the journal Nursing Outlook. While serving on active duty in the Army he achieved the Surgeon General “A” designation - considered eminently qualified to chair a department, division, or service; having attained full professional status; and have obtained national prominence in his clinical field as a Nurse Practitioner.
Dan and his wife enjoy watching their children’s sporting activities, coaching youth sports, running, golfing, traveling, and spending time with family.

I will stay with Dr. Parek as my doctor as long as I can however I will never allow another practitioner in the room

Everything was great except other than I have been waiting a week for my anti inflammatory to be called in as Dr. Was concerned with it being OK with my methotrexate? I just want to start these meds to start feeling better
