How SelfCheck Works

  • Check with Your Provider

    1. Check With Your Provider

Check with your healthcare provider if you can use SelfCheck. If so, they will place an order for a SelfCheck kit that you can pick up at a Cleveland Clinic Pharmacy.

  • Confirm Your Information

    2. Confirm Information on Your Kit

When you get your kit, make sure your name and date of birth are correct on the tube’s label.

  • Collect Your Sample

    3. Collect Your Sample

Collect your sample by swabbing both nostrils with the same swab. (Refer to the instruction sheet included with your kit or the instructions PDF for step-by-step directions.)

  • Close Up Your Sample

    4. Prepare Your Sample for the Lab

Break off the tip of the swab into the tube. Tightly screw the cap on, then place the tube into the biohazard bag. Place the biohazard bag and test order into the SelfCheck return envelope.

  • Return Your Kit

    5. Return Your Kit

Return your kit on the same day you collected it to a SelfCheck Kit Drop-Box at a Cleveland Clinic Express Care Clinic or Pharmacy near you.
