Functioning for Life Appointments

Functioning for Life is a robust 10-week program of shared medical appointments designed for new patients to the Center for Functional Medicine. Disease-specific programs have been developed and patients can select the series that is right for them.

These group appointments introduce patients to the functional medicine approach and provide the necessary laboratory testing, health information and dietary guidance needed to achieve optimal wellness. Each week, patients will interact with a multidisciplinary team of functional medicine providers including physicians, physician assistants, nurse practitioners, dietitians, health coaches and behavioral health therapists. Patients will also have access to individualized support and on-going communication with members of their care team throughout the 10-week program.

Disease-Specific Programs

Enroll in one of the following programs based on your condition:

Cardiovascular Individuals with hyperlipidemia (high LDL cholesterol, low HDL cholesterol and/or high triglycerides), with or without known cardiovascular disease. Individuals with congenital heart conditions, arrhythmias, pacemakers, recently diagnosed DVT or pulmonary embolisms do not qualify for the program.
Diabetes Individuals with metabolic syndrome, pre-diabetes and diabetes. Individuals currently on insulin therapy do not qualify for the program.
Gastrointestinal Individuals with conditions such as IBD, IBS-C (constipation), IBS-D (diarrhea), GERD/Reflux and other gastrointestinal issues.
Immune/Autoimmune Individuals with one or more autoimmune disorders such as lupus, psoriasis, rheumatoid arthritis or Hashimoto’s thyroiditis.
Osteoarthritis Individuals with conditions such as degenerative joint disease or degenerative osteoarthritis, characterized by pain in joints due to the breakdown of cartilage. This program does not address osteoporosis, therefore individuals with osteoporosis do not qualify.
Women’s Health Individuals with conditions such as PCOS, PMS, dysfunctional uterine bleeding or those in menopause.
Weight Management Individuals with a Body Mass Index (BMI) greater than or equal to 25.0 who do not have metabolic syndrome or diabetes. Individuals with anorexia nervosa or bulimia do not qualify for the program.

Each weekly program is one hour long, with the exception of sessions 1, 3, 5 and 9, which may last up to two hours. Discounted parking is available from the parking desk.
