How To Make An Appointment
You can call our main telephone line 330.753.6643 to schedule an appointment with one of our physicians. Our staff members will help you to schedule an appointment with our physician. We can sometimes see patients with more urgent complaints within 1-2 days of your referral, if your primary care physician notifies us of the urgency of your referral.
If you have an emergency, please call 911 for assistance.
You should check with your insurance provider to ensure that your consultation with our physician will be covered without a referral.
Your First Office Visit
Please arrive 15 minutes early to check in and allow yourself time to fill out three forms. These are Disclosure of personal health information, patient acknowledgement and consent and a medical history form. Also bring with you your driver’s license, insurance card, prescription card and an updated medication list.
After your check in is completed, you will have a seat in the waiting room until you are called into the patient room.
We have television as well as a variety of reading material in the waiting room for your convenience. We also have unrestricted Wi-Fi capabilities throughout our facility for your convenience. Our doctor will see you shortly after you are checked into the patient room.
After your consultation with the physician has concluded, you will proceed to the check out desk. There, you will make future appointments, get paperwork for your labs and prescriptions if needed. You will also schedule endoscopic procedures, if required. You will receive written instructions for the preparation for your procedure, if needed. You will be required to make a payment for any copay that your insurance may mandate. Your appointment will then conclude.
