Description
Portomesenteric venous thrombosis (or clotting of vessels that lead from the spleen and gastrointestinal system to the liver) after laparoscopic bariatric surgery is a rare but potentially lethal complication whose pathophysiology is not well understood. There is a disproportionately increased risk of portomesenteric venous thrombosis following laparoscopic sleeve gastrectomies that is not seen with laparoscopic Roux-en-Y gastric bypasses. We hypothesize that small changes in the size of these blood vessels or the flow through them back to the liver may be contributing to this. We wish to measure these vessels using ultrasound before and after your operation to see if this may be a contributing factor.
Inclusion Criteria
Patients at our Bariatric Surgery Center of Excellence fulfilling NIH criteria for bariatric surgery and planned operation of laparoscopic Roux-en Y gastric bypass (LRYGB) or a laparoscopic sleeve gastrectomy (LSG) as the ++primary++ bariatric procedure will be evaluated for possible inclusion.
Exclusion Criteria
1- Age <18 or >65
2- BMI <35 and > 55 kg/m2
3- Inability to walk (bed-bound or wheelchair dependence)
4- Operative time > 4 hours
5- State of dehydration marked by severe nausea or vomiting, poor PO intake, or admission to the ER in the immediate post-op period for rehydration or electrolyte imbalances
6- Current smokers
7- Inability to speak English
8- Intraoperative complications or conversion of surgery to open
9- Extended hospital course beyond post-operative day 3
10- Non-diagnostic ultrasounds during either the pre-operative or post-operative study
