Description
A multi-disciplinary technology-based approach for transitioning patients from the hospitalization for diabetic ketoacidosis to outpatient care.
Inclusion Criteria
- Age 18 years old or older
- Diagnosis of Type 1 Diabetes
- Admitted to the hospital with confirmed DKA treated with IV insulin infusion
- No prior use of CGM
- Compatible smart phone with the Libre 3
- Allergy to any component of the Libre 3
Exclusion Criteria
- Condition which is investigator judgement would limit their ability to participate safely
