Description
The purpose of this study is to investigate the quality of life in patients diagnosed with cardiac disease, including hypertrophic obstructive cardiomyopathy (HCM), thoracic aortic aneurysm (TAA), and radiation induced heart disease (RIHD). If you agree to take part, you will be asked to take three Cardiac Quality of Life Surveys over the course of nine months starting from the date you sign this consent. The first survey will be taken either in-person on an iPad provided by the Cleveland Clinic on the day you sign this consent or remotely on a personal device via a secure link that is sent to your email. The second and third surveys will be sent to you via email for you to take at home on your personal electronic device at 3 months and 9 months after your initial survey.
Inclusion Criteria
Any patient with a confirmed diagnosis of HCM and/or TAA or RIHD being seen at CC Main Campus for a comprehensive clinical cardiac evaluation (i.e. physician, surgeon, nurse practitioner, geneticist, testing, imaging, etc) to define treatment plan for either diagnosis as per CC provider. (This evaluation may occur on the same day or over a period of several days).
Patient or an individual in their social support network has the equipment (phone, computer, tablet) and internet connection to complete the emailed follow-up questionnaires.
Exclusion Criteria
Previously had surgery or plans to have surgery (full/mini sternotomy or endovascular) in the next nine months for HCM or TAA
