And now, with today’s technology, you can receive hospital-level medical care in the comfort of your own surroundings.

Cleveland Clinic Hospital Care at HomeSM brings healthcare and equipment directly to your home. After an evaluation in the emergency department (ED) or with a medical unit in the hospital, our team will decide whether this program is a good fit for you. If it is, we’ll coordinate everything and set you up with the care you need — right at home.

What Is Cleveland Clinic Hospital Care at Home?

Hospitals offer expert care and the latest technology. But home offers something just as important — comfort, familiarity and peace of mind. Hospital Care at Home gives you the best of both worlds. You’ll get the high-quality care you need in a setting you know best.

If you’re admitted to a participating hospital in Ohio or Florida, your care team will evaluate your condition to see whether you can safely receive hospital-level care at home. To be eligible, you must live within 25 miles of a participating hospital.

You may qualify if you have a condition like:

Or if you’re recovering from general surgery.

Getting care at home can make recovery feel easier and more restful. All while keeping you connected to your Cleveland Clinic team, 24 hours a day, seven days a week. You’ll have virtual and telephone visits with your care team alongside in-home visits with our skilled healthcare providers. Your at-home services may include:

  • Lab testing
  • Medication management
  • Mobile imaging (like X-rays and ultrasounds)
  • IV treatments
  • Medical equipment (such as hospital beds, oxygen, shower chairs and grab bars)
  • Physical and respiratory therapy

Hospital Care at Home is covered by Medicare, Medicare Advantage and many commercial insurance plans, so you can focus on feeling better — in the comfort of home.

A Team Approach to Care

Team-based care is at the heart of all we do at Cleveland Clinic. Your Hospital Care at Home team includes physicians, nurse practitioners and nurses. It also includes experts like respiratory, physical and occupational therapists, if needed. Your primary care provider is part of your team too, helping everyone stay in sync so your care flows smoothly at home.

What To Expect With Hospital Care at Home

Once you qualify, we’ll bring hospital-level care to your home and make sure you have everything you need to stay in touch with your care team. You’ll get:

  • A computer tablet for video visits
  • A phone that connects you directly to your team
  • A personal emergency response bracelet
  • Vital signs equipment tracked through a telehealth platform
  • A backup power supply for all equipment

Hospital Care at Home gives you the same level of care you’d get in the hospital. That’s why, while you’re receiving treatment, we ask that you stay home.

Your care team will stay in touch around the clock through a virtual command center, so someone is always available to answer questions or check in. If an emergency happens, we’ll contact paramedics right away. You can also reach your care team instantly by pressing the button on your emergency bracelet.

When your treatment is finished, we’ll take care of removing all supplies and equipment from your home. Your primary care provider will step in to manage any ongoing care.

If at any point you feel Hospital Care at Home isn’t right for you, we’ll help you transition to another form of treatment — always keeping your comfort and safety first.
