One of our top priorities is discharging the patient back to their previous living arrangements in the community after their rehabilitation stay. The percentage of patients who returned home to the community following rehabilitation was measured and compared with a national benchmark from MedTel Outcomes, LLC. MedTel Outcomes does a 90-day follow-up with patients discharged from rehabilitation facilities. They set national benchmarks based on their database of these rehabilitation facilities.

In 2022, CCCHR had 2 children (6%) that had to be discharged to long-term care due to their medical complexities.
Educational Setting Post Discharge (N = 37)

Another goal for our patients is to be able to return to the school environment to continue their education. In 2023, 75% of our patients were able to return to a school-based setting.
Preparation for Discharge (N = 48)

Parents were surveyed regarding how prepared they felt prior to their child's discharge. Parents were asked, "Did you feel prepared to have your child discharged from the hospital?"
In 2022, the survey scale ranged from a 1-5 scoring to where 1 = Strongly Disagree, 2 = Disagree, 3 = Neutral, 4 = Agree and 5 = Strongly Agree.
