Insurance Verification
Our business office staff will verify your insurance coverage before your visit to ensure all information is current. Please bring your legal picture identification, your insurance card, and method of payment for any outstanding out-of-pocket expenses. If your insurance has recently changed, please allow extra time before your appointment to update your records.
As a courtesy, we file insurance claims on behalf of our patients. However, please note that you are ultimately responsible for all charges related to your procedure at Cleveland Clinic Surgery Center, Stuart. Any questions or disputes regarding coverage should be addressed directly with your insurance company or employer.
You may receive separate bills from your gastroenterologist, anesthesiologist, pathologist, or laboratory depending on the services provided during your procedure.
Financial Responsibility
Cleveland Clinic Surgery Center, Stuart maintains its own billing policies, separate from your physician’s office. Your procedure will be billed to your insurance plan; however, you are responsible for any in-network or out-of-network deductibles, co-pays, and co-insurance as determined by your insurance carrier.
All co-pays and known balances are due on the day of service. Remaining balances are due within 90 days of service.

Self-Pay Patients
If you do not have insurance coverage, payment in full is required on the day of your procedure.

Estimated Fees
Our staff will provide an estimate of your out-of-pocket costs prior to your procedure based on the information available from your insurance provider. These estimates are not final and may vary depending on your physician’s procedure report and other factors.
You will be notified of your estimated responsibility by phone or letter before your procedure.

Additional Bills You May Receive
Because several providers are involved in your endoscopic care, you may receive separate bills from:
- Your gastroenterologist
- The anesthesiologist — Team Health
- Pathology services — Informed Diagnostics (if tissue samples are taken)
- Laboratory services — Quest Diagnostics (if applicable)
Each provider maintains independent billing and payment policies.
