Types of Services We Provide

In many cases, your family medicine physician is your first contact for long-term health and wellness. You can get all the basic care you need under one roof from our trusted team of providers.

Our family practice services include:

  • Chronic disease management: Treatment and specialty care coordination for illnesses such as diabetes, high blood pressure, arthritis, respiratory diseases and heart disease.
  • Mental health services: Screening and referrals for mental health conditions such as anxiety, depression, mood disorders and substance abuse.
  • Minor procedures: In-office tests and procedures, including joint injections, abscess drainage, EKG and imaging exam referrals.
  • Preoperative assessments: Thorough assessment of health risks and chronic disease management before any surgery.
  • Preventive care: Routine services such as annual check-ups, screenings, vaccinations, flu shots and healthy lifestyle coaching.
  • Routine gynecological care: Pelvic exams, pap smears, breast exams and family planning counseling.
  • Sick visits: Evaluation and treatment of acute (short-term) illnesses such as colds, flu, fever, bronchitis and respiratory infections.
  • Transitional care management: Assistance for people requiring care and their families during the transition from the hospital to the next appropriate care setting. The next setting may include services such as hospice or palliative care, rehabilitation or home care.
  • Treatment of minor injuries: Diagnosis and treatment of concerns such as sprains, strains, cuts, bruises, sports injuries and back pain.

Shared medical appointments

Shared medical appointments (SMAs) allow 6 to 12 people to gather in a supportive group to discuss common health concerns.

A family medicine physician leads SMAs, but you have the freedom to select the topics for discussion. Other providers, such as registered dietitians and pharmacists, may attend SMAs to answer your questions.

SMAs are entirely optional. You may benefit from a shared medical appointment if you:

  • Have general questions about health issues such as diabetes, high blood pressure, depression, nutrition or weight loss.
  • Want to connect with other people who share similar concerns and experiences.

Shared medical appointments are open to people of any age, whether they are new to Cleveland Clinic Florida or continuing care with us. The appointments last about 90 minutes and are billed to your insurance the same as a regular visit.

Shared Medical Appointments

Physicians and other healthcare professionals are always seeking better ways to offer access to high quality care to our patients. That’s why Cleveland Clinic Florida now offers shared medical appointments (SMAs).

What can I expect at a Shared Medical Appointment?

During a typical SMA, 10 to 15 patients are seen together in a setting that encourages asking questions, and sharing concerns and experiences. Patients learn from the healthcare team and from each other in this environment.

Our patients have been overwhelmingly satisfied with the program. They enjoy the opportunity to relate to other people who are dealing with similar health issues, share stories and ideas, learn from one another and truly create a bond. SMAs are particularly valuable to people dealing with chronic conditions like asthma, diabetes and hypertension.
