Request an image electronically

To retrieve a copy of your radiology images, make the request using the link below and an email will be sent when your images are ready for pick up.

Please allow at least 48 hours for all radiology image requests to be processed and for images to be available for pickup.

How to request radiology images

Please use our online request tool to request radiology images taken at Weston Hospital, Coral Springs Family Health Center and Tomsich Health and Medical Center of Palm Beach County.

To request imaging from Indian River, Vero Radiology, please follow these directions.

Please have the following information ready while making your request:

  1. Date of your exam.
  2. Cleveland Clinic Medical Record Number.
  3. Pick-up information (Weston, Coral Springs, Tomsich Health and Medical Center).
  4. Image type (MRI, CT scan, ultrasound, mammogram, bone density scan, interventional radiology or general x-ray).
