Scope of Services
Cleveland Clinic Canada is a provider of in-person and virtual primary and preventative health and wellness services. Services include, but are not limited to: (a) Medical consultations and assessments (i.e., Executive Health, Concierge Medicine, etc.); (b) Preventive care and wellness programs (i.e., via Menopause Program, nutrition services, etc.); (c) Diagnostic and screening tests, and laboratory services; (d) Health-related products and services; (e) Virtual care services. Note that virtual care services are not intended to replace in-person care and are subject to provider discretion and regulatory requirements.
Healthcare Providers
Only regulated health professionals (i.e., physicians, physiotherapists, registered nurses, nurse practitioners, etc.) may provide medical services through Cleveland Clinic Canada. Many of these providers are independent contractors who use Cleveland Clinic Canada’s platform to deliver care. Cleveland Clinic Canada acts as an agent – not as a principal – in facilitating your access to the services they provide. The relationship between you and a provider is a health professional-patient relationship. Providers are responsible for the care they deliver, compliance with professional standards and applicable laws, record-keeping and privacy obligations. Providers may determine that certain services are not appropriate or permitted and may decline to provide care at their discretion. Prescriptions, medical advice, and documentation provided by Cleveland Clinic Canada’s providers are for your personal use only and must not be shared, altered or used for unauthorized purposes.
Third-Party Services
Cleveland Clinic Canada may facilitate services through third parties other than health care professionals. If you use third-party services (i.e., prescription delivery), you will be contracting directly with those third parties – not Cleveland Clinic Canada – and those third parties will be liable for any acts or omissions.
Ownership
