Diabetes Program

Education & Building Patient Confidence

Diabetes education teaches self-management skills to control diabetes for a productive and satisfying life. Our comprehensive diabetes management program, available to inpatients, outpatients and the community, is endorsed by the Ohio Department of Health as a "model program" and is a recognized education program of the American Diabetes Association.

The diabetes healthcare team consists of your doctor, certified diabetes educators, who are nurses and dietitians, and yourself. The program consists of:

  • Inpatient diabetes education services provided by certified diabetes educators
  • Monthly diabetes group classes
  • One-on-one group teaching sessions with diabetes educators. Topics include: Healthy eating, being active, monitoring, taking medications, reducing risks, coping and problem solving
  • One-on-one teaching sessions with registered dietitians, certified in diabetes education, detailing meal planning, diet modification and weight control
  • Free community diabetes education programs, offered twice annually, with guest speakers who discuss aspects of diabetes care and self-management

Before your first diabetes education session, please print and fill out the following two forms and bring them to your first class.
