Many patients that suffer from chronic medical conditions may have difficulty achieving their treatment goals or have ongoing questions about their medications. At Cleveland Clinic in Florida, we provide access to pharmacists through our Pharmaceutical Care Clinics. These pharmacists work collaboratively with your provider to assist in your medical treatment and with patients to get medication questions answered quickly. Patients who qualify can schedule an appointment with one of our pharmacists, in-person or through our virtual platforms.

Within our Pharmaceutical Care Clinics, pharmacists will work with patients to:

  • Ensure medications are working effectively and safely.
  • Learn simple and useful tips to keep track of your medications.
  • Assist in achieving personal health and quality of life goals.
  • Reduce medication-related emergency department visits.
  • Assist in selecting cost-effective medications.

How do I qualify for the Pharmaceutical Care Clinic Program?

If you have one or more of the below conditions, and/or need assistance with your medications, ask your primary care physician for a referral to one of our primary care pharmacists:

  • Diabetes.
  • Hypertension (high blood pressure).
  • Hyperlipidemia (high cholesterol).
  • Heart Failure.
  • Asthma.
  • Chronic Obstructive Pulmonary Disease (COPD).
  • Tobacco use.

What are the program services?

Pharmacists will work closely with your doctor and healthcare team to help you meet treatment goals. Our primary care pharmacists are ready to provide qualifying patients with a number of services including:

  • Medication management.
  • Disease education.
  • Medication counseling.
  • Drug therapy problems.
  • Assistance with medication-related side effects.
  • Issues with medication costs.

How do I make an appointment?

Once you have a referral from a Cleveland Clinic physician, you can schedule an appointment at your doctor’s office.

What should I bring to my pharmacy consultation?

  • All current medication bottles and packages, including over-the-counter medicine, vitamins, supplements and herbal remedies.
  • A list of your allergies and any medications that cause a bad reaction.
  • Home monitoring devices such as a blood pressure monitor and glucometer.
