Community Health Needs Assessment Reports
Cleveland Clinic completed community health needs assessments (CHNAs) for each hospital facility. The goal of these assessments is to identify and evaluate health-related needs in the communities Cleveland Clinic hospitals serve and to use this information to fulfill our mission. The accompanying Implementation Strategy Reports (ISRs) outline the hospitals’ strategies to address identified community health needs. All reports have been formally approved and adopted as required by IRS regulations.
We invite you to view the reports for our community hospitals below:

Broward County
PACE Center for Girls Broward
Pace Center for Girls has been transforming the lives of girls and young women through a successful and proven evidence-based model, focused on the development of critical life, health, and academic skills. At Cleveland Clinic, we collaborated with the PACE team to offer the girls a program called the Healing Power of Art. This initiative allowed the young adults to process their emotions and express themselves through creativity. The girls designed stunning dresses and took part in a friendly competition to showcase their creations. Check out their incredible work here:

House of Hope
We have had the privilege of partnering with the House of Hope for several years as they work to expand their food insecurity programs. Through initiatives such as the Traveling Nutrition Garden, nutrition classes, and the impressive hydroponic gardens, their focus remains on providing healthy food to those in greatest need. The House of Hope team is dedicated to ensuring that everyone has access to nutritious meals.
Boys and Girls Club of Martin County
The Boys and Girls Clubs developed the Readers Theater as an integral component of our Healing Power of Art program. Throughout the program, students engaged in creating costumes, props, and backdrops while discovering how art, music, and theater can serve as effective tools for processing emotions and managing stress. The program culminated in a performance, followed by a dinner where students had the opportunity to share their work with their loved ones in attendance.
St. Lucie County
PACE Center for Girls Treasure Coast
The PACE Center for Girls of the Treasure Coast has served more than 1,000 girls from St. Lucie County by providing holistic social, academic, and career-building services. Cleveland Clinic collaborated with Pace’s Day Program teachers which provides year-round academics, counseling, life skills, and career preparation to create a unique Healing Power of Art & Music program that incorporated music, poetry, and visual art as an outlet for the girls. In addition, the Cleveland Clinic team of volunteers helped to transform their space with paint, new furniture, and inspiring décor.
Boys and Girls Club of St. Lucie County
Our team collaborated with the clubs to enhance their STEM programming through the introduction of Makerspace Carts—interactive learning stations that bring Science, Technology, Engineering, and Math to life in an engaging and accessible way for all children. These stations help build confidence, foster problem-solving skills, and promote teamwork, offering educational experiences that go beyond traditional learning methods. This partnership plays a vital role in ensuring that every child has the opportunity to succeed.
Indian River County
The community of West Wabasso is a historic African American residential neighborhood, with roots dating back to the 1880s. According to the 2000 census, the median household income in the area was $6,250, and residents faced challenges in gaining recognition, resulting in many issues remaining unresolved. The zip code 32970, encompassing West Wabasso, has since been abolished, complicating efforts to measure income levels, social and behavioral health, and chronic disease indicators.
In partnership with the Florida Department of Health, VNA, and GlobeChek, we offered free health screenings for adults 18 and older at the DOH Wabasso Center, formerly known as the Dasie Hope Center. Residents of Wabasso, along with individuals from surrounding communities, participated in this inaugural event.
Regional
