How to Request an Appointment
  1. Fill out the online form. Before you begin, please allow ample time and have the following information ready:
    • the patient's insurance card
    • the patient's occupation place of employment, including address and phone number
    • the patient's primary care provider name and contact information
    • Cleveland Clinic Medical ID number (if you are a current patient)
  2. Once the form is submitted, you will receive a response via email that includes the appointment time and other relevant information. You can also check the status of your request and ask follow-up questions on a secure website. The link to the website will be included in the email.
