Lodi Hospital provides skilled nursing and rehabilitation care for patients after surgery, stroke and trauma. We are an intermediate step between an acute hospital stay and going home. With shorter hospital stays, a patient's need for skilled rehabilitation services is an important part of the healing process. Skilled rehabilitation is especially appropriate for people who require rehabilitation and recuperation after an extended illness, serious injury or surgery.
Our Team
A Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides all nursing care, and patients receive expert care beyond that which is typically provided by in some skilled care facilities. The hospital's team of physical, occupational and speech therapists, social workers, nurses and physicians meet patients' needs efficiently and compassionately and work with each patient one-on-one. The team concentrates on helping patients reach their maximum potential for physical, mental, social and spiritual well-¬being. Our full-service facility including on-site laboratory and radiology is able to handle the medical needs of the patient and manage any post-acute care issues that arise.
Arranging Admission
Patients are referred from a variety of sources. The discharge planning team works with the patient and family members to find the best location to fit the patient's needs. Patients requiring skilled care services can be referred from a hospital, nursing facility and occasionally from home. Insurance plans vary for skilled care coverage and admission requirements, and Lodi Hospital staff can assist with navigating the pre-certification and admission process. The average length of stay for skilled patients is seven to 10 days, and our team works with patients and families with the goal of transitioning patients back home.
