How do I get access to my medical records at Cleveland Clinic?

For you, as a patient, there are processes in place to allow you to review, comment upon and make amendments to your medical record. Your bedside nurse or nurse manager of your hospital unit can help you with this upon request.

After you are discharged or at the end of your care episode, you may request copies of the finalized patient record. Contact Health Data Services at 216.444.2640 for more information. Hours are Monday - Friday, 8 a.m. - 5 p.m.

Portions of your medical record may be available to you through a secure, online service called MyChart. To sign up, visit mychart.clevelandclinic.org. To learn more about MyChart, select the tab above.
