Overview
Many patients require short-term nursing care and rehabilitation after a hospital stay, especially after joint replacement or when recovering from another complex surgery or illness. If a rehab facility (or “skilled nursing facility”) is your first stop before heading home, take your Cleveland Clinic care with you.
With this service, a Cleveland Clinic physician or nurse practitioner will visit you daily while you’re in rehab to continue the world-class care started at Cleveland Clinic. They will direct and supplement the care you receive from therapists and nursing staff at the nursing care and rehabilitation facility.
