With the Cleveland Clinic Connected Care Program, Cleveland Clinic medical professionals continue the expert medical care patients received at Cleveland Clinic once they are discharged to a skilled nursing facility in the community.
Cleveland Clinic physicians, nurse practitioners or physician assistants will see patients participating in the Cleveland Clinic Connected Care Program every day, five days a week, giving them and their families invaluable peace of mind. Because Cleveland Clinic medical professionals are determining patients’ course of care and working with the nursing staff at the skilled nursing facility, our patients can continue following their specialized care paths designed to help them make the best possible recovery.
While in one of the select skilled nursing facilities, Connected Care Program patients receive close medical supervision from Cleveland Clinic staff physicians, physician assistants and nurse practitioners and their medical information is documented in the same electronic medical record that was used when they were hospitalized. Patients can also benefit from Cleveland Clinic’s state-of-the-art “distance health” technologies to connect to our world-renowned team of sub-specialists remotely.

Cleveland Clinic’s goal in providing medical care at skilled nursing facilities is to improve patient outcomes. The Connected Care Program allows us to continue to monitor our patients’ daily progress once they leave the hospital. By facilitating effective communication between patients and their physicians, we can help our patients enjoy an improved quality of life and a smooth transition from skilled nursing facility to home or the next level of care.
To Enroll in the Program or Find Out More
Patients interested in continuing their care at a skilled nursing facility under the medical care of a Cleveland Clinic physician simply need to tell their hospital case manager prior to discharge. A nurse liaison will then speak with them about the program and answer questions.

Patients have the right to choose which skilled nursing facility they would like to utilize and are not obligated to continue their care through Cleveland Clinic’s Connected Care Program.
