Lakewood Hospital SeniorCare Services is dedicated to meeting the growing needs of older adults. The team provides a variety of services at multiple levels of care that benefit the healthy aging and the specialized care needs of frail seniors.
SeniorCare offers an extensive range of programs to enrich lives and provide guidance for families and caregivers.
The programs and services are available within the hospital and at our Community Health Center located at 1450 Belle Ave. (across from the hospital's main entrance) and at additional locations throughout the west shore area.
SeniorCare Assessment Center
Our expert staff provides comprehensive outpatient services for older adults experiencing a change in their physical, psychological or social well-being. The team works with older adults, their family members and the patient’s primary physician to develop and individualized care plan.

Cleveland Clinic Adult Driver and Rehabilitation Program
Adults who have experienced any physical, neurological, visual or cognitive change in function that has an impact on driving performance may benefit from a driving assessment. Our goal is to help individuals return to independent driving, as long as they are safe to do so.
The Cleveland Clinic Driver Evaluation and Rehabilitation Program provides a clinical and on-the-road assessment of driving skills, with recommendations to the client, family and physician.
Clinical Assessment Testing, determined by the National Transportation Safety Board (NTSB), includes:
- Vision function
- Visual perception
- Cognitive function
- Strength and range of motion
- Reaction time
The on-the-road evaluation takes 45-60 minutes and is completed in a Cleveland Clinic vehicle adapted for driving evaluation.
