Welcome to Cleveland Clinic Independence Family Health Center, conveniently located on Rockside Road in Independence.
Our patient-friendly facility offers a variety of physicians and specialists, offering medical care close to home or work for routine or urgent health problems. Our team of Independence Ohio Primary Care Physicians and specialists offer a variety of services, from allergy and rheumatology to orthopedics and psychology.
If you need additional services, the Independence staff can facilitate your referral to well-known community specialists or a staff member at Cleveland Clinic’s main campus, located two miles east of downtown Cleveland.
Cleveland Clinic Independence Cancer Center is Relocating
Cleveland Clinic’s Independence Cancer Center, currently located at 6100 West Creek Road, is moving to the Independence Family Health Center at 5001 Rockside Road, Crown Centre II.

The patient-centered medical home is a model of care where patients have a direct relationship with a provider who coordinates a cooperative team of healthcare professionals. The team always includes caregivers and the patient. Your medical team is invested in your care. Caregivers on your medical team include Primary Care or Specialty Physicians, Community Based Providers, Nurse Practitioners, Physician Assistants, Medical Assistants, Registered Nurses, Care Coordinators and Pharmacists. Cleveland Clinic Express Care provides no appointment necessary, walk-in medical convenience for common health problems. Express Care at Independence is located on the first floor of the the Cleveland Clinic Independence Family Health Center. Free surface and handicapped parking is available.
