Longitudinal Care Center

The Longitudinal Care Center provides care to patients with one or more chronic or long standing diseases that requires on-going treatment over a period of time. It is designed to meet their many needs in one setting. With one central location and phone number for services, patients experience ease of access and stay connected with all their caregivers (physicians, nurse practitioners, RN’s, pharmacists, social workers and clinical specialists) who get to know them as they journey through the healthcare system over an extended period of time.

There are five key services that currently make up the Longitudinal Care Center:
