The Geriatric Evaluation and Management (GEM)* at Euclid Hospital is a consultative resource for older adults, family members, caregivers and primary care physicians (PCPs). It provides a comprehensive geriatric assessment of an older adult's health issues, in the context of their social and family needs. GEM also helps with access to appropriate community and private support, and provides education and information about common geriatric health problems and concerns. *Formerly known as Geriatric Assessment Program (GAP).

The Geriatric Evaluation and Management (GEM)* at Euclid Hospital has an interdisciplinary team that includes:

  • A registered nurse.
  • A social worker.

Symptoms to watch

Older individuals in need of a comprehensive assessment could be experiencing any combination of the following symptoms:

  • Multiple health problems.
  • Confusion or memory loss.
  • Behavioral change, including depression or anxiety.
  • Difficulty in performing daily activities.
  • Weakness, caused by deconditioning or other health problems.
  • Nutritional concerns, including unexplained weight loss.
  • Problems resulting from multiple medications.
  • Uncertainty about one's ability to live independently.

Program cost

Fees for the Geriatric Evaluation and Management program are covered by Medicaid/Medicare supplemental insurance that Euclid Hospital bills directly. You are only responsible for a deductible and/or co-payment. Physicians and other specialists may bill separately through referral, but coverage of additional services will be reviewed with patients and families/caregivers before being obtained.

Take 185th St. until it dead ends at Lake Erie.
Euclid Hospital R Building will be to your right, attached to the west side of the hospital.

Park in the main hospital parking lot.
Parking is free and handicap access is available.
Valet parking is available at the hospital main entrance for a fee.
