Cleveland Clinic’s web based health management system, Dr.Connect, offers physicians full access to patient information and the ability to track patient care during evaluation at Cleveland Clinic Florida. To assist physicians in referring patients to the most appropriate and skilled provider available, we also publish Treatment Outcomes annually.

Improving patient care is the fundamental goal of our organization. We believe that publishing Treatment Outcomes and embracing innovative tools such as electronic medical records and Dr. Connect will foster better care for all patients.

Registering and Scheduling a New Patient

New patients are individuals who have never registered or scheduled an appointment at Cleveland Clinic Florida.

  • The following information will be requested:
    • Patient’s full name
    • Address
    • Date of birth
    • Social security number
    • Primary phone number
    • Alternative phone numbers
    • Patient’s employer if applicable
    • Emergency contact and phone number
    • Insurance information
    • Type of physician the patient is being referred to
    • Patient’s reason for office visit
    • Preferred date and time of office visit

Scheduling an Existing Patient

Existing patients are individuals who are already registered with the appointment center at Cleveland Clinic Florida.

Patient Self-registration and Scheduling

For patients registering and scheduling their own appointments, the process is very similar. The only difference is that both new and existing patients should call the appointment center. We ask that patients do not use the Referral Concierge Line. This line is dedicated for physician and physician staff use.
