Coverage
Cardiology testing and consults are covered by provincial health plans. The Cardiac Prevention Program is not covered by provincial health plans, though may be partially or fully covered by extended health insurance plans. Coverage for each carrier is different. Check with your plan to see what coverage is available.
- Assessment & Testing
- You will begin by meeting with our cardiologist who will review your health history, order tests, assess your present condition and prescribe a plan of action.

- Echocardiogram

- Ambulatory B/P monitor
- Primary and secondary prevention
- Therapeutic implementation
- Once assessed, you will be provided a custom lifestyle plan of meal planning and exercise.
- You will meet with your case manager once a week, either by phone, email or in person, to discuss your concerns and review your program journal.
- Maintenance and follow-up
- At the mid-point of the program, you will meet with the cardiologist for a Carotid Intima Medial Thickness test -- a measure of your cardiovascular risk factors and its predictive of coronary artery disease.
- As you draw near completion of the program, we will repeat your blood work to check levels and measure improvement.

The Primary Prevention Program is designed for persons who understand their current lifestyle contributes to cardiac risk and are ready to make a positive change.

Candidates for the Primary Prevention Program have been identified as having one or more cardiovascular risk factors:
- Smoking
- Diabetes
- Hypertension
- Metabolic syndrome

- Sedentary lifestyle
- Family history of heart disease
The Secondary Prevention Program is designed for persons who have suffered a cardiac- related condition and are seeking therapies to implement positive change in their lifestyle.
We will work with you to minimize or reduce your risk of further health implications and design an individual lifestyle plan that will improve your quality of life.
If you:
- Have stable coronary artery disease
- Have had an angioplasty or stent procedure
- Have undergone heart surgery
- Have suffered heart attacks.
The following is a sample plan of our Cardiac Prevention Program. This will be customized once the right program is determined.
Week 1
- Meet with our cardiologist to discuss medical concerns, risk factors and goals
- Undergo blood tests and other lab work
- Meet with the case manager for the first of your weekly communications
Week 2
- Meet with the exercise physiologist and dietician to create exercise and nutrition plans
Week 3 -5
- Begin two-week trial period of diet and exercise plans
Week 6
- Meet with the cardiologist for any necessary program adjustments
- Meet the behavioural therapist to discuss stress management
- Conduct CIMT (Carotid Intima-Media Thickness) test (a safe and painless ultrasound procedure that measures the thickness of the first two inner layers of the carotid artery wall. This is a direct measurement of vascular disease – the cause of heart attack and stroke – even in its early stages, before it starts to block blood flow in the artery and cause symptoms).
Week 7-9
- Continue to implement diet, exercise and stress management plans
Week 10-11
- Meet the dietician, exercise physiologist and behavioural therapist to track progress
- Re-check of blood tests and other lab work
Week 12
- Final session with the cardiologist and case manager to review gains and progress made
- Follow up session with the dietician
