Cleveland Clinic Center for Rehabilitation is your partner in helping to maximize your function and mobility after a procedure during a hospital stay or an injury. We are a specialized, subacute skilled nursing facility (or SNF).
To contact us, please call the unit at 216.445.3900.
There are various reasons, ranging from joint replacement to organ transplant, as to why you might need inpatient, specialized physical therapy or occupational therapy before going home from the hospital. Cleveland Clinic Center for Rehabilitation is ready to help.
We are a 59-bed facility on the main campus of Cleveland Clinic with a host of therapists and specialized rehabilitation equipment. We understand you are eager to return home, where you are most comfortable. If your physician has recommended after hospital, or subacute care, consider choosing Cleveland Clinic Center for Rehabilitation.
You will receive 24-hour nursing care that is monitored by a Cleveland Clinic physician, and additional Cleveland Clinic specialists are just minutes away if you need urgent medical care. Based on your needs and capabilities, the best mix of therapy services are provided, with the goal of helping you recover and stay independent.
Length of Care
The time that you spend at the Center for Rehabilitation varies depending on your needs, the rehabilitation progress that is made, and other factors.
As your discharge date approaches, your care team will review your progress and needs. For example, you may be eligible for home therapy and certain medical equipment. If you need extra care at home and live in Northeast Ohio, you may choose Cleveland Clinic Home Care Services. But regardless of where you live, the rehabilitation social workers will make all the arrangements to ensure you receive excellent care after you return home.

Cleveland Clinic Center for Rehabilitation is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) for long-term care and is licensed by the Ohio Department of Health.
