Care coordination is a vital and needed aspect to today’s healthcare services. When care is coordinated well there is a marked improvement in the transmission of healthcare information between care providers and appropriate follow up care with physicians and other care providers. This can result in reduction of medical errors, hospital admissions, and avoidable emergency admissions. This improves the patient’s ability stay within their home setting with family support.

Patients are more complex and have more comorbidities that make care for them more difficult and challenging even for the healthcare provider but with a better coordination of their care through the portals of healthcare we can improve patient outcomes.
