Cleveland Clinic’s Medicine Institute has been recognized as a Physician Practice Connections® – Patient Centered Medical Home™ (PPC® – PCMH™) by the National Committee for Quality Assurance (NCQA). Earning this recognition places Cleveland Clinic among an elite class of health care providers.
Patient Centered Medical Home is a model of care meant to strengthen the physician-patient relationship: focusing on the development of long-term doctor-patient relationships instead of episodic care. The model includes a physician-led team that provides all the patient’s health care needs and, when necessary, arranges for specialty care with other qualified physicians. A medical home also improves the medical experience for patients by offering flexible scheduling and expanded hours.

In northeast Ohio, many hospital systems have already begun to incorporate medical home components such as an Electronic Medical Record (EMR) and Care Coordinators into their daily practice. Cleveland Clinic implemented EMR more than eight years ago, enabling physicians to actively collaborate and exchange information to ensure appropriate coordination of care throughout a patient’s life.
MyChart, Cleveland Clinic’s secure, online patient-centered recording tool, connects patients to personalized health information at any time. The tool alerts patients to important, institutionally-determined health-related reminders to help them better plan the details of ongoing health care, promoting shared knowledge and enhanced patient education.
Acknowledging that multiple access points are essential to a strong healthcare system, the Clinic recently implemented “same day access.” Same day appointments now account for 1/3 of the Clinic’s primary care appointments.

In order to ensure ongoing care for patients, Cleveland Clinic is implementing disease registries of patients who need regular follow-up. Members of each care team proactively reach out to patients to ensure appropriate follow-up.
Improving communication and better coordinating care between a patient’s primary care physician and specialty providers can help minimize misunderstandings, reduce potential readmission-related issues and improve the overall quality of care.
