Requesting Home Care
Although anyone may request home care, all services must be ordered by a physician. Patients, physicians or family members can request services by calling 216.444.HOME (4663) or 800.263.0403, 24 hours a day, seven days a week.
You do not have to be in the hospital to request home care services. Patients at home or those being cared for in an outpatient setting, such as the emergency room or ambulatory clinics, may also be appropriate candidates for home care services.
If you are hospitalized, your home care clinicians—in collaboration with your physician, case manager or hospital discharge planner—will provide you with the appropriate information for planning your home care services. Following your discharge from the hospital, a member of the home care team will contact you directly to introduce themselves and arrange your first visit.
If you are not hospitalized, a home care representative will telephone you to discuss your needs, explain available services and review insurance coverage options. The representative will also ask you for a brief health history. Services generally start within 24 hours from this initial call, as long as a doctor’s order is available.
Home care services are covered by Medicare, Medicaid and most major insurance plans. For your convenience, our staff will contact your insurance carrier to determine your exact coverage. You will be informed of the extent of this coverage and any potential financial liability before services begin.
Your Home Health Care Team
Your home care team includes the staff who will be visiting and providing direct care to you, as well as a host of support staff at the home care office, all of whom work closely with your doctor to plan, monitor and evaluate the outcomes of your care. In this way, your progress can be assessed regularly and your treatment plans modified as needed, assuring you the appropriate care to speed your recovery.
