Cleveland Clinic provides the full range of services designed to meet your individual needs. For example, if you need physical therapy as your recover from knee surgery, our physical therapist will be there to help you get back on your feet. If you need help understanding you diagnosis and medications, or managing your blood pressure, our nursing professionals will assist you.

Generally, clients qualify for home care services if they:

  • are under the care of a physician who will provide an order
  • require skilled services from a nurse or therapist
  • have care needs that are intermittent, rather than continuous
  • are unable to leave home except on a limited basis

Services that are available include:

Skilled Nursing

Includes physical assessments, medication monitoring, health instruction for patients and their families and collaboration with your physician for an individualized plan of care.

Therapies (includes physical, occupational and speech)

  • Physical Therapy teaches a patient how to prevent or manage a health condition and helps motivate the patient during their treatment so they can function optimally. Physical therapists apply proven techniques to help people get back in motion. They are trusted health care professionals with extensive education and clinical experience who examine, diagnosis, prevent and treat conditions that limit the body’s ability to move and function in daily life.
  • Occupational Therapy focuses on helping people of all ages regain, develop or master everyday skills in order to live the most independent, productive and rewarding life possible. An occupational therapist will complete a comprehensive home evaluation and performance skills assessment. The findings guide development of a customized treatment program to improve one’s ability to perform daily activities. This may include recommendations for adapting the environment for safety, use of adaptive equipment and patient education, as well as guidance to family members and caregivers.
  • Speech-language Pathology encompasses the full range of human communication and its disorders. Through evaluation and diagnosis of speech, language, cognitive-communication and swallowing disorders, individualized treatment interventions are implemented. Your Speech-language pathologist’s expertise provides invaluable benefits to patients, families and other health care professionals, in managing problems (swallowing, communication, and cognitive-linguistic disorders) that affect people’s overall health, well-being and ability to benefit from other medical or rehabilitation interventions.

Nutritional Therapy

Specializes in nutritional therapy as part of a disease management process. Our registered and licensed dietitians perform a comprehensive diet analysis and in-home nutrition assessment. Once this has been completed, our registered dietitian will help to define nutrition goals and recommend diet modifications.

Tube-feeding patients will receive instruction and monitoring by the registered dietitian.

Medical Social Services

Medical Social Services provide a complete assessment on all patients referred for a consult. Services offered would include education, support and counseling to the patient and caregivers. Referrals to community resources are made as appropriate.

Care at Home Aides

Home health aides provide personal care in the home, which includes bathing, dressing and light housekeeping.

Our professionals are trained and experienced in home care; they include registered nurses, medical social workers, home health aides, dietitians, and physical, occupational, speech and respiratory therapists. Our service is backed by the world-class reputation of Cleveland Clinic.

Our home care specialists will educate and direct you to become an active participant in your health care. Learning about your medications, their intended actions and side effects, and tracking your response to them are just a few examples of how you will be involved in your care so that you can become as independent as possible.

If physical treatments or procedures, such as dressing changes or measuring your blood pressure, are part of your care plan, you will be taught and supported through the steps until you or your caregiver can safely perform these activities without professional assistance.

Although anyone may request home care, all services must be ordered by a physician. Patients, physicians or family members can request services by calling 216.444.HOME (4663) or 800.263.0403, 24 hours a day, seven days a week. You do not have to be in the hospital to request home care services. Patients at home or those being cared for in an outpatient setting, such as the emergency room or ambulatory clinics, may also be appropriate candidates for home care services.

If you are hospitalized, your home care clinicians — in collaboration with your physician, case manager or hospital discharge planner — will provide you with the appropriate information for planning your home care services. Following your discharge from the hospital, a member of the home care team will contact you directly to introduce themselves and arrange your first visit.

If you are not hospitalized, a home care representative will telephone you to discuss your needs, explain available services and review insurance coverage options. The representative will also ask you for a brief health history. Services generally start within 24 hours from this initial call, as long as a doctor’s order is available.

Cleveland Clinic Care at Home services are covered by Medicare, Medicaid and most major insurance plans. For your convenience, our staff will contact your insurance carrier to determine your exact coverage. You will be informed of the extent of this coverage and any potential financial liability before services begin.

Cleveland Clinic Care at Home serves the following counties: Ashtabula, Columbiana, Cuyahoga, Erie, Geauga, Huron, Lake, Lorain, Mahoning, Medina, Portage, Stark, Summit and Trumbull.

Cleveland Clinic Care at Home is accredited by the Joint Commission on Accreditation of Health Care Organizations (JCAHO) and is certified by the state of Ohio to participate in the Medicare/Medicaid program. We are the preferred home care provider for Cleveland Clinic Health System Hospitals, which include Cleveland Clinic, Euclid, Fairview, Hillcrest, Huron, Lakewood, Lutheran, Marymount and South Pointe hospitals.
