Our team works closely with Cleveland Clinic Health System hospitals, family health centers and physicians; this allows our patients peace of mind that their care will be seamless whether they are being discharged from the hospital and need our services or have suffered an injury at home. For our patients’ convenience we work with insurance companies if there are any questions about the coverage. Our team of nurses, therapists, aides, technicians, pharmacists and social workers provide quality care in a safe environment and assist with physical strengthening, education and medical care. Services provided at home include: nursing care, infusions, respiratory therapy, speech therapy, behavioral services, physical therapy and occupational therapy. We also offer hospice services if you choose to no longer have curative medical treatments.

Cleveland Clinic Selected to Participate in Federal Independence at Home Demonstration

The Centers for Medicare & Medicaid Services (CMS) Selects Cleveland Clinic at Home as One of 16 Sites for National Program to Measure Quality and Patient Outcomes

Cleveland Clinic has been selected to participate in the Independence at Home (IAH) Demonstration, a program of the Centers for Medicare & Medicaid Services (CMS) that increases access to medical care for frail and chronically ill patients through home-based primary care.

The new program will allow the CMS Innovations Center – which was created by the Affordable Care Act to test new models of healthcare delivery and payment – to work directly with Cleveland Clinic to demonstrate the effectiveness of comprehensive, at-home primary care. During the three-year demonstration period, Cleveland Clinic will continue to provide coordinated home-based primary care for chronically ill, low-mobility patients, while working with CMS to measure the quality of care and outcomes.

If this demonstration project succeeds in improving quality measures while generating Medicare savings, medical practices will have an opportunity to receive incentive payments which will be used to further develop and support the programs and the patients they serve. This model of care could then become available for Medicare patients nationwide.

Cleveland Clinic is one of 16 healthcare organizations across the country – and the only one in Ohio – selected to participate in the demonstration.

Cleveland Clinic at Home will begin enrolling patients this coming summer and expects to measure between 200 and 400 patients a year. The new program will create a need for job growth within Cleveland Clinic at Home, including the hiring of a social workers, nurses, administrative support, pharmacy support, nurse practitioners, and physicians. The program also plans to develop and use telemedicine, remote patient monitoring, and nurse educators to help patients in addition to physician visits. Many of these services are not paid for by traditional Medicare, but may be available with this program.

The Independence at Home Demonstration will expand and build upon existing Cleveland Clinic at Home services. The program first launched in 1999, and sees more than 2,000 patients per month. In 2011, over 18,000 patients were seen by Cleveland Clinic at Home services. The services are comprehensive and cover in-home physician assessments, skilled nursing, physical therapy, speech therapy, nutritional therapy, behavioral health, hospice, palliative medicine, home infusions, and many more specialized services.
