Home Care
Home care is generally limited to patients who meet specific criteria. These requirements include: the need for skilled services from a nurse or therapist; orders from a physician who will oversee the episode of care; needs that are intermittent, rather than continuous; and circumstances under which the ability to leave home is limited and requires considerable effort. Qualification for services is determined on an individual basis and all inquiries are welcome.

Hospice
For terminally ill patients, hospice is a philosophy of care that seeks to provide comfort rather than cure. Home Infusion Pharmacy
Home Infusion Pharmacy offers a safe and effective alternative to inpatient care. Cleveland Clinic’s licensed pharmacy provides infusion services to patients in their own homes. Patients who require infusion therapy may have diagnoses ranging from infections; multiple sclerosis; cancer and cancer-related pain; gastrointestinal diseases or disorders, such as Crohn's disease or esophageal cancer, that result in nutrition-related problems; congestive heart failure; and immune disorders.

Requesting Cleveland Clinic Home Care Services
Although anyone may request home care services, all services must be ordered by a physician.
The initial request for home care services begins with a phone call to 216.444.HOME (4663) or toll-free 800.263.0403 from a patient, physician or family member. Our staff can be reached 24 hours a day, seven days a week.
You do not have to be in the hospital to request home care services. Patients at home or those being cared for in an outpatient setting, such as the emergency room or ambulatory clinics, may also be appropriate candidates for home care services.
Paying for Care
Medicare, Medicaid and most major insurance companies cover most home care services. For your convenience, our staff will contact your insurance carrier to determine exact coverage.
