Fellowship Program Information

Clinical Fellowship In Breast Diseases

The Breast Center in conjunction with the Department of General Surgery offers a one year fellowship in breast diseases. Participating fellows must have completed a general surgery residency and be board eligible/certified as determined by the American Board of Surgery. The Breast Center provides an ideal environment for advanced training in breast disease. There are approximately 10,000 annual visits to the surgeons in the Breast Center.

Clinical responsibilities will include evaluation of outpatients with new breast problems as well as follow-up of former patients who require surveillance for either benign conditions or breast cancer. In the course of managing patients, the fellow will work closely with breast imaging radiologists, radiation and medical oncologists, breast pathologists, and plastic surgeons. The fellow will present cases at weekly multidisciplinary conferences and the Department of General Surgery Morbidity and Mortality Conference. It is expected that the fellow will complete a clinical research project during the fellowship year. Applications are accepted in the Fall each year for the following July.

Flexible Endoscopy and Laparoscopic Surgery

The Department of General Surgery offers a flexible endoscopy and laparoscopic surgery fellowship of one year in length. Applicants must have completed a general surgery residency and be board eligible or board certified in general surgery as determined by the American Board of Surgery. The fellow, in conjunction with the general surgery residents, will be primarily responsible for the care of all laparoscopic and endoscopic surgery patients on the service. This will include initial evaluation of patients, preparation for proposed procedure, conducting the procedure in a supervised fashion, and following the patient afterward. As mastery is demonstrated, the fellow will then help teach these procedures to the resident staff. It is anticipated that the fellow will participate in 100-150 laparoscopic procedures, 400-500 upper and lower endoscopies, and 120-150 ERCPs. This experience will not only ensure mastery of advanced laparoscopy and endoscopy, but also meet the recommendations of the American Society for Gastrointestinal Endoscopy (ASGE) requiring over 100 ERCPs to be adequately trained in this procedure.

In addition to clinical responsibilities the fellow will also be involved in research. This will entail both clinical trials and basic science work including the development of novel endoscopic and laparoscopic techniques in the animal laboratory. Participation will include involvement in ongoing studies as well as establishing original research protocols. The fellow will present the results of these studies at national meetings and submit manuscripts to peer reviewed journals.

Advanced Laparoscopic Bariatric Surgery Fellowship

This program is a combined clinical and research fellowship, with heavy emphasis on the science and surgical management of morbid obesity. This program is designed to produce academic leaders in the fields of laparoscopic and bariatric surgery.The program is characterized by a high volume of laparoscopic bariatric cases, including gastric bypass and laparoscopic band placement. This is in addition to ample laparoscopic foregut,colon, abdominal wall, and solid organ cases. There is an active interest in the modern management of GERD, including novel diagnostic modalities and endoscopic and laparoscopic treatment of this disease. Graduating fellows have typically completed over 200 cases, each.

Research is an integral component of the program. Fellows are strongly encouraged to conduct clinical studies and present results at national meetings. Each clinical fellow is expected to submit at least one manuscript as first author for publication. Fellows in the 2-year track enjoy close collaboration with colleagues in the basic sciences and molecular biology departments. There is a well-equipped and staffed facility for animal experiments.

Teaching skills can be refined through numerous academic workshops and courses that are sponsored by the department. Fellows participate in preparation and delivery of lectures, and in hands-on instruction of attendees. These meetings afford opportunities to cultivate long-term professional and collegial relationships with world experts in their respective fields.

Hepatobiliary and Pancreatic Fellowship

The Hepatobiliary and Pancreatic Surgery Fellowship is a one year fellowship at the Cleveland Clinic. This fellowship is designed for board eligible finishing residents and chief residents in general surgery who desire clinical experience in liver, biliary and pancreatic surgery and portal hypertension surgery. The fellowship will consist of clinical duties including patient evaluation, operative experience, post-operative management, and patient follow-up. In addition to conventional liver and pancreatic cases, the fellow will participate in the liver transplant program, both on donor and recipient procedures as well as learn the preoperative evaluation and postoperative management of these patients.

The fellowship teaching responsibilities will include involvement in regular and multi-disciplinary conferences and rounds in addition to teaching in the outpatient areas, on the ward and in the operating room.

Research responsibilities will include clinical research protocols and retrospective chart reviews and clinical trials. The fellow is expected to prepare several papers, posters and abstracts for publication and possible presentation at national and local meetings.

Endocrine Surgery

The Department of General Surgery offers an endocrine surgery fellowship of one year in length. Applicants must have completed a general surgery residency and be board eligible or board certified in general surgery as determined by the American Board of Surgery. The fellow, in conjunction with the general surgery residents, will be primarily responsible for the care of all endocrine surgery patients on the service. This will include initial evaluation of patients, preparation for proposed procedure, conducting the procedure in a supervised fashion, and following the patient afterward.

As mastery is demonstrated, the fellow will help teach these procedures to the resident staff. It is anticipated that the fellow will participate in over 200 cases. There is a special emphasis on complex thyroid cancers, reoperative parathyroid surgery, familial endocrine syndromes, laparoscopic adrenal and pancreatic surgery and thermal ablation of hepatic metastases from endocrine tumors. This unique position will afford a fellow a large surgical experience of complex cases providing an excellent opportunity for a final career in endocrine surgery.

In addition to clinical responsibilities the fellow will also support research initiatives covering both clinical trials and basic science work including the development of novel endocrine techniques in the animal laboratory. Participation will include involvement in ongoing studies as well as establishing original research protocols. The fellow will present the results of these studies at national meetings and submit manuscripts to peer reviewed journals. Applications are accepted in the fall for the following July.

ASTS Multi-organ Abdominal Transplant Fellowship

The transplant fellowship program at the Cleveland Clinic has been designed to provide a comprehensive experience in the pre- and post-operative care of the transplant patient. This broad exposure includes medical and surgical management in renal, pancreas, hepatic and other visceral transplantation. Medical evaluation and selection of recipients, surgical technique, immunosuppression, critical care management, as well as dealing with medical and surgical complications, are stressed to all transplant fellows. With almost 350 abdominal organ transplants being performed each year, and a correspondingly high number of organ retrievals, including living donation, the operative experience for surgically oriented fellows is intended to develop pre-eminent transplant surgeons. Cutting-edge procedures, such as laparoscopic living donor nephrectomy, living donor hepatectomy, split-liver procurement, intestinal and multivisceral transplant and pediatric transplantation, help to develop transplant surgeons with broad surgical exposure. In addition, a large number of ancillary procedures, such as decompressive shunt surgery, liver resection, ablative liver procedures and intestinal reconstructive procedures, train our fellows in other important diagnostic and therapeutic options.

During the first year, fellows are taught to manage patients with end-stage organ failure. First year fellows rotate for 4 months in kidney/pancreas transplantation, transplant ICU and liver/intestinal transplantation. They are taught donor procurement and kidney transplantation as well as laparoscopic living donor nephrectomy. During the second year, fellows rotate for 4 months on the hepatobiliary surgery, liver/intestinal candidate service and the liver/intestine service. During this time, they assume operative responsibilities in liver and pancreas transplantation. They also participate in the living donor hepatectomies.
