First time MyChart users will need to go through the following "Seven Easy Steps to Activate Your Personal MyChart Account".

- Click the "Activate your MyChart Account" button.
Step 3
- Enter the received "Access Code"
- Enter last 4 digits of Social Security Number
- Enter Date of Birth
- Click on the "Next" button
Step 4
- Create a MyChart ID (Do Not Use Access Code)
- Create a Password (Must Type Twice)
- ID and Password can't be the same
- Must be a minimum of 7 characters and maximum of 12
- Must contain both letters and numbers
- ID and Password should be easy to remember
and impossible for others to guess
- Create a Password Hint

- Verify or enter current E-Mail Address
- Click on the "Sign In" button
Step 6
- Read the Terms and Conditions
- To continue click the "Accept" button to agree with the Terms and Conditions
- Terms and Conditions will display each time you log on to MyChart unless you check the box marked "please do not show this page next time".
Step 7
- Click here to visit the MyChart login page and enter your MyChart ID and your Password to access your MyChart account.
- Make sure you write down and save your MyChart ID and Password Hint in a secure place.
Important MyChart User Tips!
- Once you've created your MyChart ID and Password, discard the MyChart Activation letter that contains the Access Code. You will never use the Access Code again.
- Ask your physician's office for a MyChart Wallet Card to record your MyChart ID and Password Hint.
- Bookmark or add the page from Step 7 to your favorites for quick access.
